Job summary
The purpose of the Health and Wellbeing Delivery Manager is to provide comprehensive management to the CLPCN Leadership Team, provide strategic direction for CLPCN Health and Wellbeing services and to ensure operational delivery supports the achievement of PCN contractual requirements. You will be providing direct management support to three of our health and wellbeing teams.

Main duties of the job
- oversee PCN meetings and administration support.
- Contribute to the development of information, documentation and resources across CLPCN including the production of strategies, schemes and policy documentation.
- Order goods, supplies and services as and when required, ensuring value for money.
- Line management of teams identified including 1-1s, appraisal and agreeing weekly work plan.
- Staff training and development
- Manage the development and production of relevant reports, documentation and briefing papers for a variety of key internal and external stakeholders.
- Undertake relevant research, development and monitoring activities that contribute to the development of strategies, schemes and policies.
- Manage Health, Wellbeing and Social Prescribing (HWB&SP) on behalf of CLPCN Leadership Team
- Lead PCN agenda for Tackling racial and health Inequality
- Lead on Complete Care Community initiatives
- Ensure delivery compliance for internal and external regulations and standards requirements.
- Support the PCN team to achieve contractual targets
About us
Central Liverpool Primary Care Network (CLPCN) is a group of nine GP practices, working with others, with the aim of improving health and wellbeing and addressing health inequalities in central Liverpool.
CLPCN’s shared care statement lays out our aims as follows:

We will do this by building collaborative partnerships with colleagues in the health and social care sector and will work to ensure the long-term sustainability of general practice and primary care which we believe is crucial to the wellbeing of our community.”
CLPCN have been working with our patients, staff and commissioners to understand what are the key local health challenges across our practices, and where we can make an impact. In response to this we have identified three priority areas, which are: Alcohol, Mental Health, and Supporting Children and Families. CLPCN are also committed to tackling racial and health inequalities.
