Job summary

An exciting opportunity has arisen for a hybrid role for a Phlebotomist/Care Co-ordinator, to be based at Prudhoe Medical Group on a full time basis.

The role will involve 14 hours (2 days per week) of phlebotomy clinics to support our nursing/GP team diagnose and treat illness quickly and efficiently.

The further 23.5 hours (3 days) will be spent supporting the clinical team to proactively identify and work with patients, including the frail and elderly, and those with long term conditions to provide proactive, person centred care planning, helping co-ordinate care by bringing together different specialists for the individuals needs.

Responsibilities include a wide range of patient contact and administrative duties and the provision of general support to the multi-disciplinary team

Main duties of the job

The successful candidate will play a key role in proactively identifying and working with people, including the frail/elderly/children and families and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

They will work closely with GPs and practice teams, making sure that appropriate support is made available to people; supporting them to understand and manage their condition and ensuring their changing needs are addressed. They will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve peoples quality of life.

The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

The successful candidate will have excellent interpersonal and communication skills, and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles including direct contact with people, families or carers

The post holder be expected to integrate into the nursing team for the phlebotomists role and into the wider care coordinator team within West Northumberland PCN

As a phlebotomist, you will work as part of the wider nursing team within the surgery. You will be expected to work at pace, sometimes with challenging or nervous patients and be confident with technology.

Care Co-ordinators act as a central point of contact to ensure that patients receive the best possible care, and the person is supported to achieve the outcomes that are important to them. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

To support people in preparing for or following-up clinical conversations they have with primary care professionals to enable them to be actively involved in managing their care and supported to make choices that are right for them. You will use knowledge of health and social services available in the locality, including those offered by the community and voluntary sector, to link people up with these and help them overcome any barriers they might encounter. The aim is to help people improve their quality of life and avoid unplanned hospital admissions.

The role also includes encouragement of screening, routine and planned, and assisting the patient with all concerns and outcomes along the way. An empathetic and sympathetic approach is a must for this role.
