Job summary

Our practice nurse is relocating so we are looking for an enthusiastic, experienced and caring Practice Nurse, with excellent clinical skills and warmth, with a commitment to positively contributing to our provision of high-quality patient centered care.

Main duties of the job

Ideally the candidate should have experience in General Practice.

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, clinically delivering evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. You will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of procedures.

Queenstown Road Medical Practice is a well established 7700 patient  practice.  We have a diverse demographic, situated at the north end of Battersea and pride ourselves in prioritising patient care. We have strong work ethics and work well as a team.

You will be part of an established and professional team of 7 GP's, a HCA, a full compliment of administrative staff alongside PCN staff including Pharmacists, Health Coach, Physiotherapists, Social Prescribers and Mental Health workers.

Job responsibilities

Work as part of the Nursing team to assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Assist in and perform routine tasks related to patient care as directed by GPs

Working alongside the GPs to manage Chronic Disease including Asthma, COPD, Diabetes, Heart Disease etc 

Contraception.

Cytology

Wound care /Ulcer care/ Compression 

New Patient and NHS Healthchecks

ECGs

Venepuncture

Hypertension Management

Routine & Childhood immunisations

Travel Vaccinations

Following clinical protocols

Participation in administrative systems in the Practice
