Main duties of the job
You will be responsible for reviewing and coding clinical information from patient records and correspondence, ensuring data is accurate, consistent, and up to date within SystmOne.
The role involves processing incoming information, identifying key clinical details, and supporting efficient workflows that enable safe and effective patient care. You will work closely with clinicians and the wider administrative team to ensure records are complete, relevant, and accessible.
You will also support data quality initiatives, contribute to reporting requirements, and help maintain high standards across the practice. This role requires strong attention to detail, good organisational skills, and the ability to manage workload effectively in a busy environment.

The post holder will be responsible for reviewing, summarising, and coding clinical information within patient records using SNOMED CT in SystmOne.
The role supports the delivery of safe and effective patient care by ensuring that clinical data is accurate, complete, and up to date. The post holder will work as part of the Patient Services Team, contributing to efficient workflows, data quality, and reporting requirements.
Key Responsibilities
Clinical Coding & Record Management
- Review and summarise patient records and clinical correspondence
- Accurately code clinical information using SNOMED CT within SystmOne
- Ensure patient records are complete, accurate, and up to date
- Identify relevant clinical information and escalate where necessary
Correspondence & Workflow Management
- Process incoming clinical correspondence (electronic and paper)
- Allocate and workflow information to appropriate clinicians or teams
- Support efficient document management processes
Data Quality & Reporting
- Maintain high standards of data quality and consistency
- Support QOF, enhanced services, and patient recall systems
- Assist with audits, searches, and reporting requirements
- Contribute to data quality improvement initiatives
Team Working & Communication
- Work collaboratively with clinicians and administrative colleagues
- Communicate effectively regarding coding queries or data issues
- Support a positive and professional team environment
Information Governance & Compliance
