Job summary

We are looking to recruit a Practice Nurse to join our current team of 12 Doctors, 6Nurses and 3 HCAs

We can offer either a Full Time position at 37 Hours per week or a Part Time position

We are a friendly, well established, forward thinking practice in Leeds based in twomodern purpose built premises in Leeds 9 & Leeds 15, focussed on delivering high quality care whilstmaintaining a good work life balance.

We are looking for an enthusiastic, motivated nurse, committed to having an active rolein helping to deliver and develop the health care for our large patient base. You wouldbe working as part of a friendly, team spirited group of professionals committed to providing the best service for our patients.

Our practice population have a wide variety of needs from management of complexandchronic conditions to health screening and advice, immunisations andvaccinationsand identification of treatment of all common conditions.

Main duties of the job

As a practice nurse you will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention.

Providing specialist care for registered patients in the following key areas: Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraception, Baby immunisations. Ideally you will already have experience and skills to manage one or more long term conditions. Full training and career development is available for the right candidate.

Managing the nursing care of patients with chronic disease, utilising personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes.

Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.

Providing routine treatment in general clinics, including: Wound Management, Blood tests, and ECGs.

Job description

Job responsibilities

The post holder is responsible for the delivery of general practice nursing to the practice population.

This includes the delivery of evidence- based practice for patients with long term conditions and the provision of preventative healthcare.

As an autonomous practitioner the post holder will demonstrate critical thinking and skills in clinical decision-making in the management of patients. Working in accordance with the NMC Code of Professional Conduct, they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policies and procedures, and provide nurse leadership and support as required.

Clinical Responsibilities

  • Deliver treatment programmes for patients with a known long-term condition, or those at risk of developing a long term condition.
  • Work with the full Practice team to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
  • Review patient needs and treatment holistically in accordance with evidence-based practice and national and practice protocols
  • Individualise care and involve patients in decisions to support the delivery of person-centred care
  • Prioritise health problems and intervene appropriately to assist in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support the care and review of patients in relation to asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, adult and childhood immunisations, weight management, mental health and learning disabilities.
