Job summary

An exciting new opportunity has arisen for us to recruit a Digital and Data Lead (full time, or 4 days) to join our rapidly expanding primary care team, working across Havant & Waterlooville Primary Care Network (PCN). The Network supports over 67,000 patients registered with our five member practices across the area.

The post holder will be an integral part of the PCN management team and will work across all network services and with member practice teams.

The role offers the opportunity to use your IT knowledge and skills to support improvement in access to general practice through greater understanding and use of digital solutions, as well as contributing to service efficiency and deveopment by the timely production and interpretationof performance data.

Main duties of the job

Purpose of the role

The Digital and Data Lead is the PCN champion of all things data and digital, to enable accurate and timely reporting of service performance and to explore and recommend, where appropriate, the adoption of new hardware, software or processes for network services or practice operations.

This will include generating performance data and working with the Operations Manager and PCN service team leads to understand performance indicators and whether these could be more useful, and time-limited project work, focusing, for example, on specific processes and exploring whether technology, collaborative working, training or a simple workplace change might improve service efficiency or outcomes.

Part of the PCN leadership team, the PCN Digital and Data Lead will use clinical records, appropriate software and public health data to monitor, understand and report on PCN and, where relevant, network member practice performance and reach.

About us

Havant and Waterlooville PCN has five member practices* stretching from Denmead in the north-west of the area to Havant and the villages beyond in the south-east. Together the practices have a patient population of approximately 67,000. The PCN serves an area with pockets of significant deprivation. The population served by member practices is older than the national average.

*Network member practices:Blossom Healthcare, Bosmere Medical Practice, Denmead Practice, Oaks Healthcare, Village Surgery

The network exists to support practices in serving their patients to improve health and wellbeing. This includes operating services that work better at scale and working with practices to explore areas of common interest.

Partners from three of our practices share the Clinical Director role; the practice managers from all practices and a lead GP from the other two practices are also routinely involved with PCN discussions/decisions.

The PCN team is currently 40 people, including the Network Manager, Operations Manager and admin support, and patient-facing teams:

  • social prescribers and care co-ordinators
  • dementia supportincluding an Admiral Nurse and health & wellbeing coaches
  • mental health supportincluding an Adult, and a Children's and Young Person's mental health practitioner, health & wellbeing coaches and mental health advisors
  • clinical pharmacists
  • first contact musculo-skeletal practitioners
  • home visiting paramedics
  • proactive care nurse

Part of the PCN leadership team, the PCN Digital and Data Lead will use clinical records, appropriate software and public health data to monitor, understand and report on PCN and, where relevant, network member practice performance and reach.

The role requires engagement with PCN, practice and ICB colleagues to understand current service models, identify challenges, agree if data or digital improvements are appropriate and then prioritise evaluating these and supporting their introduction.

Working with the network manager, operations manager, clinical directors, service team leads and practice managers, the post-holder will develop strong relationships and have the confidence to support the introduction of new technology and processes and the ability to engage with and inspire colleagues in all roles across the network to adopt agreed new ways of working that will benefit patients and staff.

The Digital and Data Lead may share line management responsibility for some administration colleagues with either the Operations or Network manager.

Key Responsibilities and Duties

Data management and digital transformation

Use data and improve data quality to understand service demand, capacity and activity and to drive improvements in patient access, operational efficiency, and staff experience

Identify digital technology that can improve service delivery and patient outcomes and optimise the use of digital tools within service delivery and performance monitoring/ management

Work with the operations and/or network manager to formulate and present business cases for the purchase of and/or use of this technology

Ensure, where possible, cross-organisational IT solutions are researched, procured and deployed across all the PCN/member practices

Support the adoption of new technology and other initiatives to enhance service delivery and patient access to care

Enable PCN and, where appropriate, practice staff, to work more effectively to support the sustainability of general practice services via the promotion of PCN services and the NHS App, the use of cloud telephony, digital triage and online consultations

Collate data to monitor service performance against all PCN DES specifications and other relevant indicators as agreed

Service management

Work with service team leads and, where appropriate, practice managers, to support development of initiatives to improve effectiveness, efficiency and quality of operational processes to ensure the smooth running of PCN, and where appropriate, practice, services

Support the PCN team leads to develop appropriate IT understanding and skills within their teams and train new staff as required

Support the effective adoption of national and local data and technology-related initiatives as agreed by member practices

Responsibility for PCN electronic assets

PCN management activity

Provide data/business intelligence to enable the management team to formulate strategic plans for, for example, workforce, estates, services (including capacity and access development) and digital infrastructure

Lead on the design and delivery of key performance data sets for the PCN.

Prepare reports on behalf of the PCN for commissioners and other organisations as required. This will include but is not limited to reports on the contractual requirements under the PCN DES, Enhanced Access and Home Visiting Service contracts and Capacity and Access reporting

Responsible for PCN website and social media channels and support practice web and social media channels as required

Work with the network management team and member practices to agree content for PCN /practice websites and social media / digital communications, enhancing the profile of the PCN and engaging the wider community

Support the adoption of new technology and other initiatives to enhance patient access to care.

Support the corporate operation of the PCN, working with the Network Manager, Operations Manager and network administration colleagues, practice managers and clinical directors to ensure meetings and operational working groups are effectively managed, agreed actions minuted, logged and delivered
