Job summary
Admiral Nurses are specialist dementia nurses who work with and support people with dementia and their families who have complex needs.
You will lead Havant and Waterlooville’s Dementia Health & Wellbeing Team (a small established team of dementia-specialist Health & Wellbeing Coaches, who will underpin the work of the Admiral Nurse) across the six GP practices within the PCN and will also influence and lead on the delivery of work streams to improve the experience of care for people living with dementia, and their families.
Main duties of the job
You will be employed by the Bosmere Medical Practice on behalf of Havant and Waterlooville PCN, working under the management of the Clinical Director for the PCN and will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will also become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse Competency Framework.
