Job summary

Job Purpose

The aim of social prescribing is to help people take control of their health and well-being.

In General Practice, patients are referred to non-medical link workers who are normally known as Social Prescribers.

Social Prescribers then work with individuals to help identify what is important to the patient and to connect them to community groups and statutory services for practical and emotional support.

They also work with other agencies to help ensure support services are available, accessible, appropriate and sustainable.

In addition to helping people at the individual level, social prescribing has the potential to help reduce health inequalities and strengthen community resilience by addressing the wider determinants of health such as debt, poor housing, physical activity and peoples' involvement with their local community.

The role is therefore important at both the individual and community level.

Main duties of the job

1. Manage a caseload of referrals from PCN GP practices and other agencies.

2. Develop personalised plans with patients which reflect their priorities and enable them to improve their mental, physical and social wellbeing.

3. Develop relationships with staff in GP practices and elsewhere and contribute to multi-disciplinary meetings as appropriate.

4. Proactively identify gaps in service provision in the local area and work with others to help create new services which can help build individual and community health well being and resilience.

About us

Ribble Primary Care Network (PCN) is a federation of seven independent GP Practices in the South Preston area who are working together to improve the range and quality of services for patients.

The seven practices are:

Beeches Medical Centre / Fishergate Hill Surgery / Kingsfold Medical Centre / Longton Health Centre / Lostock Hall Medical Centre / St Fillan's Medical Centre

The post holder will work with all of the seven practices and the role may also involve home working and working at other sites.

KEY DUTIES AND RESPONSIBILITIES

1. Manage a caseload of referrals from the PCN and other agencies and work with individuals to build trust and understanding of their goals.

2. Give people time and co-develop personalised plans which reflect their priorities, interests, values and motivations to enable them to access relevant health and other services to improve their mental, physical and social wellbeing.

3. Provide support to patients, their families and carers in a way which is non-judgemental, respects diversity and lifestyle choices and helps build on and develop an individuals skills and strengths.

4. Where appropriate, help people maintain or regain independence through skill building, adaptations, enablement approaches and simple safeguards. Explore and support access to personal health budgets where appropriate.

5. Utilise relevant assessment tools to identify individuals needs, strengths and existing skills and capabilities and track the improvement in their wellbeing following the agreed interventions and support.

6. Develop relationships with staff in GP practices within the PCN, attend relevant multi-disciplinary meetings and provide information and feedback on social prescribing interventions.

7. Develop relationships with other Social Prescribers in Ribble Primary Care Network and elsewhere and with community health providers and wider public services to support the efficient functioning of multi-agency work and referrals.

8. Proactively identify gaps in service provision in the local area and work with others to help create new services which can help build individual and community health wellbeing and resilience.

9. Work within established processes and procedures and understand when it is appropriate and necessary to refer people back to other health professionals / agencies and when what the person needs is beyond the scope of the Social Prescriber role.

10. Help build clinical and non-clinical understanding across the Primary Care Network around what other services are available in the community and how and when patients can access them.

OTHER RESPONSIBILITIES

1. Contribute to the development of a culture within the Ribble PCN and its practices which puts the patient at the heart of all care and support delivery.

2. Undertake training and continual personal and professional development as required, including regular professional supervision by a GP or other clinical professional as well as personal reflection, to enable you to carry out your role effectively and safely. It is a mandatory NHS requirement that the post holder:

a. completes the NHS England online training programme https://www.e-lfh.org.uk/programmes/social-prescribing/

b. is enrolled in, undertaking or qualified from appropriate training as outlined by the Personalised Care Institute Core Curriculum and social prescribing link worker competency framework - https://www.england.nhs.uk/personalisedcare/supporting-health-and-care-staff-to-deliver-personalised-care/personalised-care-institute/

c. attends the peer support networks delivered at place or system by the ICS and /or NHSE&I in the region

3. Act in accordance with all health & safety policies, safeguarding, lone working and other statutory and policy requirements.

4. Practice good information security, observe all relevant legislation, policies and procedures and ensure good supporting documentation of interventions with patients.

5. This job description describes the main features of the role but the role is expected to evolve. The post holder may therefore be required to carry out other duties and responsibilities at an appropriate level of responsibility.
