Job summary

This is an exciting opportunity for a qualified Practice Nurse to join our friendly team at Flansham Park Health Centre. Ideally, the successful applicant will work 25.5 hours per week (hours to be agreed).

Main duties of the job

You will be responsible for chronic disease management and a wide range of treatment duties to our patients.

Ability to carry out the following competencies would be desirable as would an interest in developing in these areas:

  • Respiratory care (COPD/Asthma)
  • Diabetes

Flansham Park Health Centre is a training Practice with 4 dedicated GP Partners and 8 Salaried GP's supported by a team of highly professional clinical and non-clinical staff to provide the highest possible standard of care to over 13,200 patients.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Key responsibilities:

  • Chronic disease management
  • Treatment room services (i.e. wound care, ulcer care)
  • Cervical smears
  • Childhood immunisations
  • Assisting GPs with coil fitting/removal, implant fitting/removal and other minor surgery procedures
  • Pathology requests and diagnostic procedures
  • Participation in multi-disciplinary team and clinical meetings
  • Working as part of a team as well as independently
  • On occasion there may be a requirement to undertake home visits
  • Maintain a clean, tidy, effective working area at all times

Desirable:

  • Experience of undertaking cervical screening (smears)
  • Experience of undertaking childhood immunisations
  • Experience of undertaking travel clinics and vaccinations
  • Experience of running chronic diseases clinics
  • Experience of providing respiratory care (COPD/Asthma)
  • Experience in the management of diabetes

Knowledge and Skills

  • Excellent communication skills (written and oral)
  • Strong IT skills, including audio typing
  • Clear, polite telephone manner
  • Effective time management (Planning & Organising)
  • Ability to work as a team member and autonomously
  • Good interpersonal skills
  • Problem solving & analytical skills
  • Ability to follow policy and procedure
