Job responsibilities

The post holder is responsible for the delivery of practice nursing care to the practice population. Supported by the MDT practice team and the wider PCN team, they will deliver care within the boundaries of their role. Focusing on supporting patients to live healthy lives, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively to meet the needs of patients, supporting the delivery of policy and procedures, as well as various primary care contracts such as the PCN DES, QOF and local improvement schemes. They will also be expected to provide nurse leadership as required, to members of the wider nursing team.

Qualifications

Essential

• Registered Nurse with current NMC registration. Evidence of continuous professional development and the ability to work without direct supervision.

Desirable

• At least 1 years experience of working in general practice.
• Experience in managing long-term conditions.
• Experience of treatment room duties required for the post.
• Awareness of clinical governance issues in primary care.
• Knowledge of PGDs and associated policy.
• Experience of quality initiatives such as QOF.

Experience

Essential

• Experience of working in general practice.
• Experience of relevant clinical systems such as System1.
• Skills including : Venepuncture, ECG, wound care , adult /children vaccination and immunisation. Health Education, cervical cytology.
• Women's health and Contraception.
• Qualification / experience in running type 2 diabetes chronic disease management clinics.
• IT skills including accurate data entry.
• Demonstrate appropriate professional and personal boundaries.
• Able to demonstrate commitment to high quality care and service provision.
• Respects the privacy, dignity, needs and beliefs of patients, carers and colleagues.

Desirable

• Non-medical prescribing qualification or willingness to work towards this.
• Experience in insulin initiation in type 2 diabetes or willingness to work towards this.
• Experience in primary care
• Experience in GP practice
