Main duties of the job

As an Advanced Care Practitioner at Knebworth Surgery, you will be integral to delivering high-quality patient care. Your primary duties will include:

  1. Clinical Duties:

    • Conducting full patient assessments, diagnosing, and treating patients across a broad spectrum of conditions. You will have the support of a GP in the triage hub for advice and assistance whenever needed.
    • Although not essential, a knowledge of managing frail elderly patients and care home residents is advantageous.
  2. Collaboration and Teamwork:

    • Working closely with a dedicated team of healthcare professionals, including 3 other ACPs and 5 GPs, ensuring a collaborative approach to patient care.
    • Participating in frailty Multidisciplinary Team (MDT) meetings and practice teaching sessions to enhance team learning and patient outcomes.
  3. Patient Interaction:

    • Interacting with patients daily, providing compassionate and comprehensive care, and ensuring continuity and follow-up as needed.
  4. Professional Development:

    • Contributing to our learning organisation by engaging in teaching and mentorship activities, supporting both your colleagues' and your own professional growth.

Clinical Responsibilities:

Clinically examine and assess patient needs from both a physiological and psychological perspective, and plan clinical care accordingly through telephone triage or face-to-face consultations.
