Job summary

This role of Specialist Mental Health Practitioner for Primary Care is fundamental to the development and leadership of innovative and integrated mental health services initially based in a large GP practice covering two locations. The role promotes and provides rapid triage, early assessment, treatment, and improved access to evidence-based interventions for people experiencing mental health problems.

Main duties of the job

The post holder will join the clinical team 

Case find, screen, triage, assess and coordinate the development of care plans for people as part of the primary care team.

Provide brief, outcome focused support and time limited evidence-based interventions for people.

Offer interventions to people using the principles of motivational interviewing.

Support people to build their emotional resilience and crisis planning tools, promoting self-management skills.

Ensure systems are in place to promote smooth pathways between primary care and secondary mental health services.

Job description

Job responsibilities

The post will involve travel between the two practice sites (Knebworth and Marymead) to fulfil the role. The post holder will work alongside other clinicians in the practice on occasion as part of a multidisciplinary team. The post holder will need to be able to work autonomously to triage, assess, case manage and discharge a patient without referral to a GP where appropriate.

The post-holder must be able to advise on medications, identify and intervene to meet social needs, order any relevant investigations, and make onward referrals if necessary.

The post holder will act as the first point of contact in primary care. They will accept referrals for triage via agreed routes within the practice. They will offer rapid and autonomous ‘first contact’ triage and promote early treatment by supporting people directly into appropriate mental health pathways across both the statutory and voluntary sectors and will provide short term interventions on the telephone, via video and in person at the surgery. They will on occasion be some requirement for a home visit where both safe and appropriate.
