Job summary

We are seeking a Long Term Condition Nurse (Diabetes) to join our nursing team at Dalston Medical Group.

Our focus is on finding the correct character with excellent interpersonal skills, to support our current team in caring for all our patients, but specifically looking to enhance the care for our patients with diabetes. Evidence of professional development is essential. Whilst hours are negotiable, this post is for up to a maximum of 25 hours per week. You must be flexible to cover holidays and sickness absence.

Main duties of the job

The post holder is responsible for the delivery of general practice nursing to the whole practice population.

Working within a dedicated clinical team with a specialist interest in diabetes, you will be involved in the routine, evidence based care and education of patients with diabetes, including annual reviews.

About us

We are a small rural practice in the village of Dalston near Carlisle. Our practice list size is approximately 5100. Our clinical team comprises of four doctors, two practice nurses and two Health Care Assistants. We have a strong training ethos, hosting medical students, and we may expect staff to contribute towards teaching.

You will be required to work independently as a Practice Nurse with long-term conditions management and treatment room skills within the scope of professional practice.

Undertake the Long-term condition clinics including Diabetes primarily and chronic heart disease providing specialist nursing care for patients according to skills and knowledge.

Manage programmes of care for patients with Long-term conditions by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directives.

Excellent communication skills through patient consultation and nursing assessment of physical, psychological and social needs.

Assess, plan and implement patient centred plan of care for patients and refer when necessary to other healthcare professionals for further specialised care.

A qualification in undertaking cervical cytology is desirable, but not essential. The practice may ask you to undertake training in this area.

Assist with child immunisations and adult vaccinations.

Maintain disease registers, including an awareness of QOF and other incentive schemes.

Perform investigative procedures including; performing ECGs, Urinalysis, Blood Glucose monitoring, Venepuncture, measuring and interpreting Peak Flow.

To maintain accurate healthcare records.

Adapt to change within working situation managing unscheduled emergency situations.

Ensure mandatory training has been completed.

Maintain continuing professional development.
