Job summary

We are looking for an enthusiastic, experienced and caring Advanced Nurse Practitioner to utilise a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions. You will have excellent clinical skills and warmth, with a commitment to positively contributing to our provision of high-quality patient centred care.

With the government-lead PCN agenda we now find ourselves requiring an advanced nurse practitioner, who shares our values and has a desire to be an excellent provider of care, to join our existing highly skilled team.

Candidates will be employed on a pay rata based on the equivalent of Band 7-8 (although we are not an agenda for change organisation) depending on qualifications and experience. We encourage personal clinical progression in a nurtured environment according to need.

Main duties of the job

This role will involve assessing and triaging patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

The successful candidate will be part of a dynamic and forward thinking PCN lead multi-disciplinary team (MDT). The primary care network is made up of two local GP Practices, Blackthorn Health Centre and Hedge End Medical Centre.

We welcome applications from candidates who require flexible working patterns, for example annualised contracts or full-time hours compressed over 3-4 days.

  • NMC Registered Nurse with clinical post registration experience.
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
  • Evidence of continual professional development within the last two years relevant to advanced nursing practice.
  • Health assessment / minor illness qualification.

  • Driving license.
  • Core mandatory training achieved.
