We have an exciting opportunity at a friendly South Hampshire GP Network on the river Hamble. The Eastleigh Southern Parishes, Primary Care Network is looking to recruit an advanced nurse practitioner to join their expanding Primary Care team to enhance services for patients living in the locality. Main duties of the job

Eastleigh Southern Parishes Network Ltd is a local GP federation responsible for managing the Primary Care Network services and staff. It is an ambitious organisation owned by 2 local GP practices and a local management consultancy. We believe that through sharing our expertise we can provide improved services to our patients as well as strengthening the primary care infrastructure. We believe we have a very exciting and forward-thinking organisation which is committed to significantly delivering improvement to the quality of patient care. With the government-lead PCN agenda we now find ourselves requiring an advanced nurse practitioner, who shares our values and has a desire to be an excellent provider of care, to join our existing highly skilled team.

Candidates will be employed on a pay rata based on the equivalent of Band 7-8 (although we are not an agenda for change organisation) depending on qualifications and experience. We encourage personal clinical progression in a nurtured environment according to need.

We aim to develop accessible and high quality health services to people in Eastleigh Southern Parishes whilst supporting General practice and the NHS through collaboration with Commissioners and other providers of Health and Social Care.

Job description

Job responsibilities

To work autonomously and across the primary care network practices Blackthorn Health Centre, Hedge End Surgery and ESP Primary Care Hub in Lowford, using advanced knowledge and skills, underpinned by theory and experience.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

The post holder is expected to be able to:

Works unsupervised, clinically examining and assessing patients and initiating any appropriate action.

Working at a masters level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice, actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person, and complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.

Essential

  • Candidates must have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice.
  • NMC Registered Nurse with clinical post registration experience.
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
  • Health Assessment / minor illness qualification.

Desirable

  • Core mandatory training achieved.

Experience

Essential

  • Evidence of continual professional development within the last two years relevant to advanced nursing practice.

Desirable

  • Knowledge of EMIS.
  • Experience of working in the NHS.
  • Experience of working in GP Practice.
