Thornbury
Medical Centre looking for 2 part-time experienced Practice Nurse's to join ourvibrant and friendly Practice andnursing
team.
We pride

and have a culture in learning and teaching in all we do. We are looking for a
forward thinking experienced Practice Nurse who has the qualifications and/or
experience to support our patients with long term health conditions and be
comfortable with the ever-changing day to day world of General Practice.
Main duties of the job
Experienced in Primary Care, the applicant can expect to be involved in Chronic Disease Management (Diabetes, CVD, Respiratory Disease and of course Long Covid etc), other
Quality and Improvement Goals e.g. Learning Disability Health Checks and Mental
Health Physical Health Checks, Vaccinations and Immunisations (Adult and
Child), Cervical Screening, Spirometry, ECGs, Contraception and Sexual health,
Travel Medicine, Ear Irrigation and Venepuncture.
Good Knowledge of Clinical Governance and Infection Prevention Control
principles are essential. Good standard of written English and computer
literacy are required.
On occasion it may be necessary to undertake home visits (e.g. flu
vaccination and long term disease monitoring).
About us
We are an
average sized practice with just over 7,000 patients. GPs, Advanced Clinical Practitioner,
Pharmacist, HCA and Physician Associate and other health professionals.
We have an
admin and reception team that supports the clinical team and patients.
We are part
of Bradford PCN 5 Primary Care Network and work closely with our neighbouring practices.

Practice Manager and the Lead Practice Nurse, you will deliver care within your
scope, focus on supporting patients to be healthy, monitoring long-term
conditions, health prevention and screening activities. You will work
collaboratively with the whole team to meet the needs of patients and improve
outcomes, supporting the delivery of policy and procedures and provide nurse
leadership as required.
The post holder will also strive to maximise the practices achievement
of both Local and National Quality and Performance objectives.

- Registered Nurse with relevant Nurse Qualification / Degree.
- Post graduate education in the management of Diabetes, Asthma and COPD.
Desirable
- Current (within the last 12 months) emergency resuscitation training certificate.
- Experience of Systmone Clinical System.
Experience

- Car driver with clean licence.
- Able to work at the desired times.
- Flexible and able to cover for colleague absence through sickness, annual leave or study leave.
- Experience of primary care.
- Experience and competent in General Treatment Room Duties. Experience of working in accordance with the NMC Code of Practice. Experience of dealing with the public/patients
- Ability to work without direct supervision and determine own workload priorities.
