Mayflower Medical Group is a Primary Care practice which if forward looking and revolutionary. Based in Britain’s Ocean city, Plymouth, situated in West Devon. The area hosts Dartmoor National Park, an area of outstanding natural beauty to the north as well as the Atlantic Ocean on the our doorstep; the location offers a great balance of work life balance, and many outdoor activities are available.

Mayflower is innovative in its approach. The team are purposefully driving a transformation within primary care in terms on approach, model, and resourcing. The group is comprised of 5 sites over the city serving over 37,000 patients operating as a single primary care network. In a model change the team asked, ‘how can primary care adjust and cope with the changing environment’. As part of the wider change, we have moved to a triage first using an ACP lead framework; to enable this to be a success we are seeking a variety of staff to join the friendly team based in Devon.

We are looking for ACPs from all disciplines. We are not limiting the role to any specialty, recognising that a wide team will enable better and more appropriate care. We already have First contact Physiotherapists, Advanced Nurse Practitioners, Advanced Paramedics, Clinical Pharmacists, Health Coaches, Physician Associates as well as Social Prescribers.

Main duties of the job

The role of an ACP in primary care will have autonomy in clinical triage, assessment, and referrals. There will, of course, be GP support when required. The working week could comprise of triage or booked phone call and face to face clinics. The successful candidate could work in one or a mixture of clinic styles. There is a dedicated Paramedic home visiting team so very few visits are needed.

The Pharmacist led team manage, authorise, and monitor all repeat medications currently issuing about 80,000 prescriptions a month.

Mayflower is a SystmOne based practice, with AccuRx and eConsult as the primary contact methodology.

Mayflower Medical Group has secured training capacity for those who do not hold a NMP qualification but who would like to, as well as ACP MSc programs for the right individual.

Job Summary:

The post holder will be an experienced clinician who, acting within their professional boundaries will provide clinical assessment and management of patients who are triaged and identified as requiring a home visit by the Acute Care Team (ACT) and the Duty Doctor.

The post holder will carry out visits to patient’s private homes, as well as visits to patient in Nursing and Residential home registered within the Mayflower Group.

Supported by a mixed skilled team, they will work within the community, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for patients including assessment, diagnosis and treatment, to deliver quality patient services.

Committed to the development of innovative practice and expanding evidence-based care which will contribute to the enhancement of the Paramedic Practitioner role.

Provide and maintain a high standard of care for patients in their own homes as well as providing support to the GP’s and other members of the primary health care team.

Key Responsibility Areas:

Clinical

  • Making an assessment of their physical, psychological and social needs through focused history taking
  • Develop a clear management plan with patients including appropriate prescribing (where able or with support from the duty team) and referral
  • Know own limits in relation to scope of practice and recognise when a requirement exists to refer to, or seek advice from a more senior clinician.

Adopt an evidence based approach to patient care

  • Have a broad range of knowledge, clinical skills and competencies to enable the needs of patients to be met
  • Maintain anaphylaxis and resuscitation techniques
  • Recognise the signs, symptoms and categories of child abuse. Have an understanding of child health procedures, statutory local procedures and points of referral and the basic legal issues around child safeguarding
  • Recognise and be aware of appropriate referral mechanisms for family violence, vulnerable adults and addictive behaviours
  • Recognise psychological needs of patients with depression or suicidal tendency and respond appropriately to refer to support teams/agencies as required

Professional

  • Provide a high standard of professional conduct and nursing care at all times in accordance with the HCPC
  • Attend relevant training and education to maintain professional and clinical competence
  • Adhere to the Medicines Act of 1968 and the HCPC Guidelines for the administration of medicines
  • Work within own professional competence at all times and refer to ACT Duty Doctor as necessary
  • Ensure accurate record keeping of all consultations are maintained at all times
  • Undertake an active role in audit and research undertaken within Mayflower that will have the broad objective of improving or re-evaluating this service

  • To ensure familiarisation with disaster recovery and emergency procedures
  • Complete all paperwork and maintain administrative systems appropriate to Company needs.
  • Provide assistance to colleagues ensuring smooth operations and to provide effective responses to both individual and group needs.
