Job summary
We are looking for GPs to join our dynamic PCN multidisciplinary
team to cover 9-10 sessions. Part time welcome. (Please include in your application how many sessions you can do).
They will join our
current PCN employed GP to make a team of 3 PCN GPs. This growing GP team will
contribute to our team of teams approach that connects the 7 practice teams
with our PCN pharmacy, mental health, practice support, and social prescribing
teams.
The role will involve some sessions of practice-based GP
clinics. There will be access to senior GPs
for advice and support, and opportunity for supervision and debrief as needed.
The remaining sessions will be used to develop a PCN
based portfolio career, pursuing interests that span population health and
proactive personalised care. We have a number of PCN projects where a GP can
slot in to contribute as part of our innovative approach to metabolic health,
chronic pain, mental health or women's health. However, we also remain open to
ideas that new GPs bring, and consider other network level projects they would
like to develop
Our approach as a PCN is to work closely in partnership with
our local population, to
develop a collaborative community of support. This incorporates the new
disciplines of social prescribing, health coaching, care co-ordination
alongside a wide primary care MDT (including mental health workers, pharmacists
and pharmacy assistants, first contact physio, GP assistants and digital lead)
Main duties of the job
We anticipate this role will include (by mutual negotiation):
-4-6 sessions / week
-Good supervision and mentorship with senior PCN GPs and managers
-Opportunity to join innovate approaches to Metabolic health and Chronic pain, including group consultation, lifestyle interventions, peer support.
-Working as part of our PCN MDTs
-Opportunity to develop leadership skills (work alongside PCN Clinical Director)
-Time for personal development, and non- patient facing learning
-Mix of core General Practice clinics in practices and more innovative services (as above)

Job responsibilities
In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including:
Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home.
Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner.
Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers.
Assessing the healthcare needs of patients with undifferentiated and undiagnosed issues.
Screening patients for disease risk factors and early signs of illness.
Developing care plans in consultation with patients, in line with current disease management protocols.
Providing counselling and health education.
Admitting or discharging patients from the caseload and referring them to other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Collecting data for audit purposes.
Attending MDT meetings
Participation in education/teaching of wider MDT teams including medical and non medical teams
Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible.
Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate.
Undertaking emergency patient visits during surgery hours, if clinically indicated.
Participating in on-call sessions and home visiting sessions as allocated in the practice rota.
