Heeley Plus Primary Care Network consist of 7 practices in Sheffield, serving a population of around 42,000 patients. We are an active network who is currently actively engaged in work with our local health and social care organisations to support and develop new care models for our patients and community.

Main duties of the job
You will lead as a cancer champion for the network. Work will be done across practice sites as well as active work with our voluntary organisations and community. The candidate will be required to attend all mandatory training programs.
• To assist the Primary Care Network (PCN) in delivering improvements to the services we provide our patients in relation to the following aspects of patient care –
• Participation in national cancer screening programmes
• In support of collaborative working the post holder will form productive and supportive relationships with practice staff who have the skills, knowledge and remit to contribute to this work. e.g. GP practice non – clinical cancer champions, social prescribers and secretaries.
• The post holder will assist practices to evaluate their screening uptake and engage hard to reach populations to participate in order to reduce health inequalities. This will include working alongside practices to enhance processes to track and followup screening non-responders.
• Support the PCN’s constituent practices to implement a range of core activities and systems to improve patient take up of cancer screening. When needed to provide direct support to practices to follow-up screening non-responders.

accurate records of support.

Encourage people, their families and
carers to provide feedback and engage fully in the care coordination process.

Work closely within the MDT and with
GP practices within the PCN to ensure that the comprehensive records of MDT
case discussions are inputted into clinical systems, adhering to data
protection legislation and data sharing agreements.

to seek feedback, continually improve the service and contribute to business
planning.

Contribute to the development of
policies and plans relating to equality, diversity and health inequalities.
