Heeley Plus Primary Care Network consist of 7 practices in Sheffield, serving a population of around 42,000 patients. Main duties of the job

You will lead as a cancer champion for the network. Work will be done across practice sites as well as active work with our voluntary organisations and community. The candidate will be required to attend all mandatory training programs.

• To assist the Primary Care Network (PCN) in delivering improvements to the services we provide our patients in relation to the following aspects of patient care –

• Participation in national cancer screening programmes

• In support of collaborative working the post holder will form productive and supportive relationships with practice staff who have the skills, knowledge and remit to contribute to this work. e.g. GP practice non – clinical cancer champions, social prescribers and secretaries.

• The post holder will assist practices to evaluate their screening uptake and engage hard to reach populations to participate in order to reduce health inequalities. This will include working alongside practices to enhance processes to track and followup screening non-responders.

• Support the PCN’s constituent practices to implement a range of core activities and systems to improve patient take up of cancer screening. When needed to provide direct support to practices to follow-up screening non-responders.

We are currently growing our network team and looking for a proactive and dynamic person to join our care coordinator team with a special focus on supporting uptake, addressing barriers and health promotion of cancer screening and quality improvement work in Early Cancer Diagnosis.

Job responsibilities

Provide personalised support

§ Work with the patient, their families and carers and consider how they can all be supported by services available to them.

§ Bring together a person’s identified care needs and explore their options to meet these within a simple coproduced personalised care and support plan, including what they can expect from the groups, activities and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.

§ Seek advice and support from the Clinical Leads and/or identified individual(s) to discuss patient-related concerns (referring the patient back to the GP or other suitable health professional if required).

Data capture

§ Work sensitively with people, their families and carers to capture key information, enabling comprehensive and accurate records of support.

§ Encourage people, their families and carers to provide feedback and engage fully in the care coordination process.

§ Work closely within the MDT and with GP practices within the PCN to ensure that the comprehensive records of MDT case discussions are inputted into clinical systems, adhering to data protection legislation and data sharing agreements.

Other

§ Work as part of the healthcare team to seek feedback, continually improve the service and contribute to business planning.

§ Contribute to the development of policies and plans relating to equality, diversity and health inequalities.

§ Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

§ Directly managed by Medical Support Team leader, and accountable to Primary care network manager and Clinical Director

Desirable

  • -has experience of working in a healthcare setting

Qualifications

  • -experience in paid /voluntary or placement working in any health/care setting.
  • -previous admin and clerical experience within the NHS or similar field
  • - GCSE English & Maths grade C or above (4 or above in new scale) or Key Skills 2 Numeracy NVQ3/ GCSE grade C in English and Maths
  • -Valid Driver's Licence
  • -Knowledge and experience of using a range of data bases or patient --information systems
  • -Good organisational skills.
