Job summary

We are looking for a motivated and enthusiastic Practice Nurse to join our team at The Gables Medical Group. We are a small friendly practice who strive to support each other. The ideal candidate would work efficiently with excellent time management and communication skills. Motivational, with a friendly approachable manner.

Main duties of the job

The successful applicant will ideally be an experienced Practice Nurse with enhanced skills in managing long term conditions. Duties will include Diabetes, childhood immunisations, cervical screening, Asthma and other general practice nurse duties including health promotion and health education.

About us

Our team includes 3 GP Partners, 2 Salaried GPs, 1 ANP, 1 NP. Nursing team includes 2 Practice Nurses, 1 Nursing Associate, 1 Training Nursing Associate and 1 HCA. We have a very supportive admin team. We are part of Wansbeck PCN and work closely with neighbouring practices. PCN staff include FCP, Pharmacist, MHW, Health & Wellbeing coaches and community link workers. We are a teaching practice and support GP Registrars, medical students and nursing students. We encourage innovation and encourage ideas for change to improve the quality of our service. Teamwork is our ethos and every role in the team is valued.

Job description

Job responsibilities

Role summary: The post holder is responsible for the delivery of safe and effective nursing care to the whole practice population. Has Enhanced skills in managing long term conditions. Duties will include Diabetes, cervical screening, childhood immunisations, and other general practice nurse duties including health promotion and health education. Excellent time management and communication skills who is a self-motivator with a friendly and approachable manner. We will support further development and training.

Clinical Responsibilities:

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long-term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions

Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care

Assess and care for patients presenting with uncomplicated wounds

Support and advise women requesting information relating to family planning needs

Support and manage health needs of women presenting for cervical cytology consultations

Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad

Assist senior practitioners in providing minor-surgery sessions if required

Provide chaperone service

Carry out blood tests and urine tests when required.

Maintaining, Monitoring & Updating Computer Database and Management of Medical Records & Practice Audits

  • Ensure that practice clinical database is updated promptly and accurately with information relating to patient records, as required for clinical accuracy and for practice recording requirements under IM&T protocols, QoF, IIF and enhanced services

  • Participating in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits

Completion of Information Governance on line toolkit annually and provide certificate

Maintain current CPR and anaphylaxis certification, child safeguarding training and any other mandatory training as required.
