Main duties of the job

As a Practice Nurse, your key responsibilities will include:

  • Managing and reviewing patients with long-term conditions such as diabetes, asthma, COPD, and cardiovascular disease.

  • Delivering immunisations for adults and children, including travel vaccines.

  • Carrying out cervical screening, sexual health checks, and wound care.

  • Performing treatment room procedures, including dressings, ECGs, blood tests, and injections.

  • Supporting health promotion, lifestyle advice, and patient education initiatives.

  • Collaborating closely with GPs, healthcare assistants, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.

  • Maintaining accurate patient records and ensuring clinical governance, safety, and infection control standards are met.

  • Participating in clinical audits, quality improvement projects, and professional development opportunities to enhance service delivery.

Job Summary

The post holder is responsible for the delivery of Practice Nursing services to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job Responsibilities

Providing assessment, screening and treatment services and health education advice

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to general practitioners as necessary

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long-term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion

Support and advise women requesting information relating to family planning needs

Support and manage health needs of women presenting for cervical cytology consultations

Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad

In participation with the Nurse Manager, ensure the maintenance of equipment and stock levels relating to patient care

Pathological specimens and investigatory procedures

Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs

Administrative and professional responsibilities

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice

Ensure collection and maintenance of statistical information required for regular and ad hoc reports

Attend and participate in Practice meetings as required

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols

Mentor and training of Health Care Assistants

Research projects

Co-operate and participate as required in any research projects within the Practice

Student/staff education and training
