Job summary

We are excited to offer two fantastic opportunities to join our friendly and welcoming team at Oakhill Surgery. We are a unique, small rural practice in Somerset and we would love to offer you the chance to apply to join us. As well as our other application for a new Practice Nurse (see separate advert) we are recruiting for a Lead Practice Nurse to replace our previous longstanding nurse who has retired.

With the support of the practice management team and GP partners, as Lead Practice Nurse, you will have the chance to shape the future of the nursing service for our rural community.

Main duties of the job

As the new Lead Practice Nurse you will manage the excellent nursing care team including a practice nurse, a HCA and a GPA phlebotomist. Continuing to ensure the effective delivery of compassionate, friendly and personalised care that has always been at the heart of nursing at Oakhill Surgery.

You will be responsible for several clinical areas such as infection prevention and control, chronic disease management and various aspects of proactive health promotion, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

As part of your chronic disease role you will work as a senior member of the practice multidisciplinary team responsible for the implementation and delivery of care for patients with long-term conditions using up to date and evidence-based practice, including care for elderly and housebound patients.

Due to the nature of this work the successful candidate will have chronic-disease qualifications, such as in Diabetes, Asthma or COPD, sufficient to compliment the rest of the multidisciplinary team. We will happily consider those who are keen to expand their knowledge and qualifications in this area.

Examples of some of the additional nursing duties provided by the team would also include immunisations for adults and children, wound care, cervical screening.

We would especially welcome those with experience in female health care and family planning.

The following are an example of your core responsibilities as Lead Practice Nurse however there may be, on occasion, a requirement to carry out other tasks.

a.To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager

b.To produce the IPC annual statement in conjunction with the practice manager

c.To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

d.To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

e.To develop, implement and embed health promotion and wellbeing programmes

f.To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

g.To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

h.As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

i.As required, to diagnose and manage acute and chronic conditions

j.To prescribe medication for patients in accordance with PGDs and own scope of practice

k.To review patient medications to enhance compliance

l.To provide guidance to patients on the use of prescribed and over-the-counter medications regarding side effects and dosages

b.Develop, review and implement nursing protocols in conjunction with the partners

c.Support all members of the nursing team, providing guidance when necessary, conducting appraisals and training needs analyses as required

d.Participate in local initiatives to enhance service delivery and patient care

e.Support and participate in shared learning within the practice in order to improve patient care

f.Continually review clinical practices, responding to national policies and initiatives where appropriate

g.Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)

h.Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs

i.Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients
