Job summary

We are looking for an Advanced Nurse/Clinical Practitioner to join our friendly team for 4 sessions / 2 days per week. Working hours to be agreed with the successful candidate. Starting salary is dependent on experience.

Working as part of the clinical team covering AskMyGP queries (using email, telephone or face to face, depending on clinical judgement/preference). Share of duty sessions. Evening/early morning enhanced access work desirable.

Possibility to develop special area of interest within the practice or wider PCN/federation and neighbourhood team.

Start date 1st April 2024 but willing to wait for the right person.

Main duties of the job

Use of online demand management system (AskMyGP) to assess and manage all patients requests by working as an autonomous practitioner, independently and in conjunction with other health care professionals to assess, diagnose and treat patients, initiating referrals where appropriate.

Working alongside the GPs in undertaking home visits, taking part in the duty rota, managing pathology results and clinical documents relevant to patients seen, a share of non-specific clinical documents, covering for colleagues on leave and being a clinical resource for all members of the practice team.

Providing training education and support to others within the practice as needed, contributing to development of local policy, procedures, guidelines and patient services, and acting as a role model and occasional mentor for the practice nurse team.

Main surgery with branch surgery 5 minutes drive apart serving 9,500 patients in urban/suburban area. Spacious premises with ample staff parking.

Our large and friendly team currently includes 2 GP partners, 3 salaried GPs, 2 X Advanced Clinical Practitioners, a highly skilled nursing team, Practice Manager, Assistant Practice Manager and a large team of administrative and reception staff.

This includes: the HomeHealth dedicated care home team led by ANPs and access to a whole range of other specific roles to include clinical pharmacists; a Therapy team including OTs; Social Prescriber; Health and Wellbeing coach; Admission and Discharge Care Coordinator and Musculoskeletal Practitioner Service.

Working as part of an established, supportive clinical team to provide services between 0800hrs and 1830hrs and as part of the Primary Care Networks enhanced access programme. Enabling the practice to meet the patients needs by seeing the right person at the right time, appropriate to their needs.

Job Responsibilities:

Advanced Clinical Practice:

  1. Manage and deliver care to patients with previously undifferentiated and undiagnosed health problems, through face to face appointments, emails, telephone triage and or advice and home visits
  2. Undertake robust clinical assessment and examination of patients, diagnose and safely and successfully manage presenting conditions, some of which are complex
  3. Treat patients appropriately within local and national guidelines including prescribing in line with legislation and competency and advising about alternative therapeutic interventions
  4. Make appropriate referrals to members of the multidisciplinary team and other primary and secondary care services, both acutely and routinely
  5. Request, interpret and manage test results for patients seen
  6. Make professionally autonomous decisions for which post holder is accountable for, recognise limits of own professional practice and act on these
  7. Provide patients with evidence-based treatment that is regularly reviewed, and which incorporates cultural preferences, health beliefs and behaviours
  8. Undertake and advise on diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion and public health strategy
  9. Utilise effective communication strategies to ensure information and knowledge is imparted and recorded sensitively and maintain accurate, contemporaneous and confidential records
  10. Undertake advanced skills appropriate to the role ensuring clinical competences are maintained.

Education and Development:

  1. Identify own development needs in line with service requirement and personal development plans and undertake clinical supervision
  2. Provide mentorship for trainee ANP/ACPs, prescribers and other members of the practice team
  3. Assist in developing information for patients and the public in communicating and promoting the work of the service
  4. As a clinical expert be able to advise on educational opportunities that will facilitate the development of clinical practice

Management and Leadership:

  1. Act as a role model of standards for behaviour and profession through commitment to the ANP/ACP role, service and practice principles
  2. Maintain professional registration and practice through CPD, reflection, clinical supervision and portfolio for revalidation

Research and Audit:

  1. Contribute to evaluation of the service and further development within the practice and wider health environment as needed
  2. Undertake audit of individual practice including prescribing, share findings and participate in wider practice audits and assist with implementing changes recommended
  3. Keep up to date with current research, interpret and disseminate findings to colleagues, and amend practice accordingly

Communications and Working Relationships:

The post holder must be able to:

- Communicate complex and sensitive information to a variety of members of the multidisciplinary team and patients, requiring negotiating, persuasion, listening and influencing skills. This may include the delivery of unwelcome and adverse information and news

- Engage with patients and carers in their illness or disease management, promoting active participation in decisions about their care through informed choice and providing supportive literature and demonstrate empathy

- Demonstrate excellent information technology, numeric, literacy and verbal skills

- Act as an advocate for patients

- Liaise with GPs, practice staff, secondary and other care providers and outside agencies regarding patients, carers and families

- Take part in practice meetings as required

Essential Qualifications / Skills / Experience Appropriate to this Post:

- Registered Nurse with current NMC registration or Paramedic with current HCPC registration

- Qualified advanced / autonomous clinical practitioner including non-medical prescribing with post qualification experience (or be willing and able to undertake this very soon), ideally but not necessarily in primary care

- Evidence of very good IT and computer skills
