Job summary
We are looking for an Advanced Nurse/Clinical
Practitioner to join our friendly team for 4 sessions / 2 days per week. Working hours
to be agreed with the successful candidate. Starting salary is dependent on experience.
Working as part of the clinical team covering AskMyGP queries (using email, telephone or face to face, depending on clinical judgement/preference). Share of duty sessions. Evening/early morning enhanced access work desirable.
Possibility to develop special area of interest within the practice or wider PCN/federation and neighbourhood team.
Start date 1st April 2024 but willing to wait for the right person.
Main duties of the job
Use of
online demand management system (AskMyGP) to assess and manage all patients
requests by working as an autonomous practitioner, independently and in
conjunction with other health care professionals to assess, diagnose and treat
patients, initiating referrals where appropriate.
Working
alongside the GPs in undertaking home visits, taking part in the duty rota,
managing pathology results and clinical documents relevant to patients seen, a
share of non-specific clinical documents, covering for colleagues on leave and
being a clinical resource for all members of the practice team.
Providing
training education and support to others within the practice as needed,
contributing to development of local policy, procedures, guidelines and patient
services, and acting as a role model and occasional mentor for the practice
nurse team.

Our large and friendly team currently includes 2 GP partners, 3 salaried GPs, 2 X Advanced Clinical Practitioners, a highly skilled nursing team, Practice Manager, Assistant Practice Manager and a large team of administrative and reception staff.

face to face appointments, emails, telephone triage and or advice and home
visits
- Undertake robust clinical assessment
and examination of patients, diagnose and safely and successfully manage
presenting conditions, some of which are complex
- Treat patients appropriately within
local and national guidelines including prescribing in line with
legislation and competency and advising about alternative therapeutic
interventions
- Make appropriate referrals to members
of the multidisciplinary team and other primary and secondary care
services, both acutely and routinely
- Request, interpret and manage test
results for patients seen
- Make professionally autonomous
decisions for which post holder is accountable for, recognise limits of
own professional practice and act on these
- Provide patients with evidence-based
treatment that is regularly reviewed, and which incorporates cultural
preferences, health beliefs and behaviours
- Undertake and advise on diagnostic,
health screening, health surveillance and therapeutic interventions within
a broader health promotion and public health strategy

- Maintain professional registration and
practice through CPD, reflection, clinical supervision and portfolio for
revalidation
Research and Audit:
- Contribute to evaluation of the
service and further development within the practice and wider health
environment as needed
- Undertake audit of individual practice
including prescribing, share findings and participate in wider practice
audits and assist with implementing changes recommended
- Keep up to date with current research,
interpret and disseminate findings to colleagues, and amend practice
