The Care Co-ordinator will work closely with PCN team and support the clinicians across our 6 practices to helping patients living with long term health conditions.

The successful candidate will be caring, dedicated, reliable and person-focused and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing patients, their families and carers with high quality support

Main duties of the job

  • Proactively identify and work with patients with long term conditions, including the frail/elderly and
  • Provide navigation of care and support across health and care services
  • Focus on supporting and delivery of personalised care to reflect local PCN priorities, health inequalities or at risk group patients.
  • Assist with the network covid vaccination programme, including administrative support to clinics, booking patients and performing data entry and preparing reports as necessary
  • Prepare and run searches in the clinical system for meeting target specific goals and objectives
  • Ensure patients are contacted and directed to the right service based on network practice call/recall policy and clinical system alerts
  • Managing digital services, including practice website

We are a group of 6 practices covering the rural north and east of Winchester. The practices are at Gratton, Stockbridge, Whitchurch, Watercress, Alresford and West Meon.

We are looking for a dedicated individual to work with the PCN Practices to help provide integrated care inconjuction our GPs, Paramedics, Pharmacists, Social Prescriber and & Medical Administrators.

  • Assist and coordinate PCN clinics where necessary, including the monthly paediatric clinic, and contact identified patients with appointments and liaise with practices as appropriate.
  • Assist in the coordination of the PCN COVID Vaccination Programme.
  • Coordinate the PCN virtual Care Homes MDT, liaising with all clinical and non-clinical members, including care home registered managers, to ensure effective MDT function. Prepare background information about individuals for inclusion in the live lists for the MDT meetings.
  • Take minutes of MDT meetings and disseminate and follow up agreed actions as appropriate
  • Run audits and searches where necessary to monitor and report on PCN performance against the requirements of the Impact and Investment Fund (IIF), Quality Outcomes Framework (QOF) and other set Enhanced Services across the PCN practices.
  • Manage patient initiated calls for help, ensuring patients are directed to appropriate service
  • Document and monitor aspects of patient co-ordination and service delivery supporting data collection and audit using the practices clinical system
  • Support and raise awareness of national screening and immunisation programmes and other health promotions, encouraging participation to ensure targets are achieved at a practice and PCN level.
