Job responsibilities
The
successful applicant will be expected to include some extended hours in their
pattern of work.
Main duties of the Job
To perform as
an autonomous general practitioner working independently and with other health
care professionals to assess, diagnose and treat the conditions of patients
attending within a primary care setting, both in practice and on home visits.
In
accordance with the practice rota, as agreed, the postholder will undertake a
variety of duties including surgery consultations, telephone consultations and
queries, visiting patients at home, checking and signing/authorising repeat
prescriptions and dealing with queries, paperwork and correspondence in a
timely fashion.
Making
professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within the
organisation.
Assessing
the health care needs of patients with undifferentiated and undiagnosed
problems.
Screening
patients for disease risk factors and early signs of illness.
In
consultation with patients and in line with current practice disease management
protocols, developing care plans for health.
Providing
counselling and health education.
Referring
to other care providers as appropriate.
Recording
clear and contemporaneous consultation notes to agreed standards.
Compiling
and issuing computer-generated acute and repeat prescriptions.
Prescribing
in accordance with the practice prescribing formulary (or generically) whenever
this is clinically appropriate.
The
postholder may be required to visit patients during surgery time in an
emergency if clinically indicated.
The
postholder will undertake fair share of home visiting sessions per week as
allocated.
The
post holder will be expected to undertake all the normal duties and
responsibilities associated with a GP working within primary care, as set by
Royal College of Practitioners, General Medical Council, all other Royal
colleges and councils applicable to staff within the practice, Department of
Health CCGs and all other regulatory and standard setting organisations.
Practice
Meeting and Personal Development
The practice
is committed to the highest standards of evidence-based medicine and supporting
colleagues in achieving this. You will be expected to participate in
discussions on clinical practice standards, developing practice protocols,
mutual professional support for the individual practitioners, audit,
significant event analysis, meetings with colleagues in the locality, etc.
Where these occur on an ad hoc basis, adjustments to clinical workload may be
required.
Personal
CPD (continuing professional development) time
This may
include a mix of in-house meetings and events, time away from the practice,
either in private study, attending educational events or time in lieu of
attending educational events outside of normal working hours. Full time
salaried GPs are entitled to 4 hours per week (on an annualised basis) study
days per annum and for part-time employees this amount is adjusted on a pro
rata basis.
Trainees
You will be
expected to provide support for any students or trainees in the practice.
Practice
Development
You may be
expected to become responsible for particular areas of practice development.
QOF/Enhanced
Services
You will to
be responsible to lead on specific areas of the Quality and Outcomes Framework
(QOF).
Management of risk
Manage and assess risk within the areas of
responsibility, ensuring adequate measures are in place to protect staff and
patients. This includes the escalation of clinical concerns or appropriate
referrals where the management of a clinical situation requires it, or it falls
outside of clinical competence.
Monitor work areas and practices to ensure they are
safe and free from hazards, and conform to health, safety and security
legislation, policies, procedures and guidelines.
Apply infection control measures within the
practice according to local and national guidelines and in accordance with the
practice policy.
Managing information
Use technology and appropriate software as an aid
to management in planning, implementation and monitoring of care, presenting
and communicating information.
Review and process data using accurate SNOMED codes
in order to ensure easy and accurate information retrieval for monitoring and
audit processes.
Collate, analyse and present clinical data and
information to the team.
Other
Responsibilities within the Organisation

to service development and implementation of contract.

of and compliance with all relevant Practice policies/guidelines/procedures,
e.g. prescribing, child protection, confidentiality, data protection, health
and safety, discipline.

to individual case discussions at clinical meetings and bring examples of cases
for group discussion.
Contributing
to the organisational development of the Practice by contributing to the
development of ideas, strategies and policies which will promote efficiency and
well-being of the practice.

to evaluation /audit and clinical standard setting with the organisation.
Contributing
to the development of computer-based patient records.
Contributing
to the summarising of patient records and Read-coding patient data.
Attending
training events organised by the practice and other agencies, where appropriate.
Attending
team meetings and other meetings as may be necessary.
Confidentiality
In
the course of seeking treatment, patients entrust us with, or allow us to
gather, sensitive information in relation to their health and other matters.
They do so in confidence and have the right to expect that staff will respect
their privacy and act appropriately.
In
the performance of the duties outlined in this Job Description, the post-holder
may have access to confidential information relating to patients and their
carers, practice staff and other healthcare workers. They may also have access
to information relating to the practice. All such information from any source
is to be regarded as strictly confidential.
Information
relating to patients, carers, colleagues, other healthcare workers or the
business of the practice may only be divulged to authorised persons in
accordance with the practice policies and procedures relating to
confidentiality and the protection of personal and sensitive data.
