An exciting opportunity has arisen and we are seeking an Advanced Nurse Practitioner to work with us at Edgworth Medical Centre.
We have 4000 registered patients under our care.
Job Summary:
- The post holder will demonstrate courageous and critical thinking and is a compassionate and committed experienced nurse who, acting within their NMC professional boundaries.
- The post holder will commit to demonstrating safe, competent clinical decision-making process and expert care, including providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care; for all aged patients presenting with acute and chronic illness within the general practice.
- They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Main duties of the job
Job Responsibilities:
Clinical Practice:
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Initiate necessary invasive and non-invasive diagnostic tests or investigations sand interpret findings/reports.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
- Refer patients directly to secondary care, other services/agencies as appropriate.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients in order to support compliance with and adherence to prescribed treatments.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
About us
Edgworth Medical Centre is a small Practice based in Edgworth Bolton. We have a branch surgery at Egerton & Dunscar Health Centre. We have 4000 registered patients under our care.
We currently have the following staff working with us:
5 Salaried GP's (part time), ANP, Practice Nurse, HCA.
We also work collaboratively with Turton Primary Care Network to delivery services as a PCN including extended hours etc.
We work with the following clinical staff from the PCN:
Physician Associates, Mental Health Practitioners, MSK Practitioners, Pharmacists.
We have a friendly administration team who help with the smooth running of the surgery.
All our staff are committed to providing excellent patient centred care to our patients.

- Successfully completed Cytology smear taking programme
Desirable
- Completion reproductive healthcare (family planning ) and sexual health care courses
- Faculty of family planning diploma of Implanon and IUD/IUS procedures
- Proof of knowledge relating to menopause management
Experience
Essential
- Experience of working in health services, particularly womens health, including menopause
- Experience of working in Primary Care
- Experience of working with doctors/general practitioners and advance health care professional as part of a team but also able to make clinical decisions for diagnosis and treatment independently
- Working in a multi-disciplinary team
- Experience of different health and social care organisations
