Job summary

In collaboration with Dementia UK, Derby South PCN are looking to recruit a registered nurse with a passion for, and experience of, providing relationship-centred dementia care with a strong sense of advocacy and equalities for this client group to join Derby South PCN and launch a new community-based Admiral Nursing service.

Admiral Nurses are specialist dementia nurses who work with and support people with dementia and their families who have complex needs. As an Admiral Nurse you will provide specialist one-to-one support, guidance, and practical solutions for families, alongside supporting best practice and providing role modelling across the PCN.

You will be employed by Derby City South Healthcare Ltd and based within the practices and communities covered by the PCN. You will work under the direct management of the PCN Manager and will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse Competency Framework.

Main duties of the job

The successful applicantwill be a NMC registered nurse,withsignificantpost-registration clinical experience of working with people with dementia andsupportingtheir carers/supporters in different settings, including the primary care setting, along with formal knowledge ofdementia acquired through clinical post-graduate training/education.

As the Admiral Nurse Clinical Lead, you will be balancing the clinical caseload for the service, alongside setting-up, developing and evaluating the service.You will also influence and lead on the delivery of work streams that improve the experience of care for people living with dementia, and their families from pre-diagnosis to post-bereavement.

The postholder will work within the PCN Home Visiting Service to provide a holistic approach to anticipatory care utilising the multi-disciplinary approach where appropriate. This will involve delivering clinical 1:1 sessions for families affected by dementia, often in the patients own home.

Due to the need for home visits, the postholder will need to have a driving licence and their own transport available for work use - mileage will be reimbursed.

About us

Derby City South Healthcare Limited is the delivery vehicle of Derby City South PCN. The company currently provides the Derbyshire 'Team Up!' Home Visiting Service specification on behalf of the practices.

We employ a number of GP's, Paramedics, ECP's, Care Coordinators and Administrators to provide this service. We are looking to expand to include Admiral Nurses and other Long Term Condition Nurses, in order to provide a preventative element to the Acute Home Visiting Service.

The PCN covers seven practices and 103,000 patients in the South of Derby City and the North of South Derbyshire district:

Hollybrook; Parkfields; Village; Willington; Wellbrook; Melbourne & Chellaston; Alvaston.

We are a large and demographically varied PCN, with patient care and the sustainability of General Practice in everything we do.

We have a base office at Community Action Derby, 30 Charnwood Street, DE12GU, with plenty of parking available.

Facilitation of education and training for staff in the delivery of evidence-based dementia care

Clinical leadership with policy development and quality improvements in relation to dementia care

Consultancy to other health and social care professionals regarding dementia

Main Duties and Responsibilities:

Clinical

Work within the GP Led Complex needs team and Home Visiting Service to provide a holistic approach to anticipatory care utilising the multi-disciplinary/ multi-agency team where appropriate

Deliver regular clinical 1:1 sessions for families affected by dementia who meetthe referralcriteriawithin designated areas/ practices

Provide clinical leadership on the care of people with dementia and their carers/families with complex care needs from peri-diagnosis to post bereavement support, to support reduced reliance on GP appointments

Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework or appropriate tools for carers/families affected by dementia, including positive risk taking, managing distressed behaviours and risk minimisation strategies

Provide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families

Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers/families

Develop and support care plans for people with dementia and families including positive risk management

Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia

Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment

Ensure the safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Deprivation of Liberty Safeguards (2009)[2] are considered and adhered to in relation to patient care

Act as an advocate for people with dementia and their carers/families to promote their rights and best interests

Participate and lead on case conferences, continuing health care processes and best interest meetings where required

Work collaboratively with health, social and voluntary sector services to achieve coordinated care

Work in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families, around advance care planning, best interest decisions and end of life care

Manage and advise on complex care transitions for people with dementia and their carers/families to reduce the frequency of crisis situations

Regularly attend multi-disciplinary team meetings for long-term conditions to inform and receive information regarding patients who have a diagnosis of dementia

Refer to secondary care if there is deterioration in condition/a review of medication including antipsychotics is required or any other concerns regarding a persons health that cannot be managed in primary care, in line with their dementia expertise

Support and advise on carer assessments and reviews

Ensure that care is delivered in accordance with relevant local and national guidelines and all relevant Derby City South policies and procedures
