This is a great opportunity for an experienced Practice Nurse looking for career progression and a new and exciting challenge. Derby City South PCN are seeking a a Long Term Condition Nurse to join our PCN 'Team UP' Home Visiting Service.
The PCN provides a holistic Home Visiting Service to housebound patients within our PCN, and we are looking to add long term condition management to the range of services we provide. The successful candidate will take a key role within this team.
The PCN are looking for someone who can hit the ground running and has experience in managing long term conditions within a practice setting.
Main duties of the job
This role will include telephone appointments and home visits to meet the needs of our patients. The postholder will require transport and driving licence to visit patients within the PCN boundary.
As an autonomous practitioner the nurse is responsible for the care delivered demonstrating critical thinking and skills in clinical decision-making in the management of patients.
A good knowledge of Systm One is an advantage, as is being an enthusiastic team player with good organisational and communication skills.
About us
Derby City South Healthcare Limited is the delivery vehicle of Derby City South PCN. The company currently provides the Derbyshire 'Team Up!' specification on behalf of the practices.
We employ a number of GP's, Paramedics, ECP's, Care Coordinators and Administrators to provide this service, and are looking to expand to include LTC nurses.
The PCN covers seven practices and 103,000 patients in the South of Derby City and the North of South Derbyshire district:
Hollybrook; Parkfields; Village; Willington; Wellbrook; Melbourne & Chellaston; Alvaston.
We are a large and demographically varied PCN, with patient care and the sustainability of General Practice in everything we do.
We have a base office at Community Action Derby, 30 Charnwood Street, DE12GU, with plenty of parking available.

Ensure health and safety and infection control are high priorities, supported by other members of the team.
Work with our wider team to ensure we monitor performance of QOF and Long Term Conditions regularly to achieve high standards of patient care.
Perform Long Term Condition Reviews in the patients own home.
Undertake routine immunisations.
Take responsibility for own development, learning, and performance, including organising clinical supervision where appropriate and acting as a positive role model.
Work with colleagues and management team to ensure that we focus our clinical time to maximum benefit of patients and that our systems and processes support staff to focus on what matters most.
To administer medication for patients in accordance with PGDs and own scope of practice
Wound care management
Request pathology services as necessary
To process pathology results as required
Perform a holistic assessment of patients for cervical cytology smear tests
To maintain accurate clinical records in conjunction with existing legislation
Ensure read codes are used effectively
Prioritise health issues and intervene appropriately.
To support the team in dealing with clinical emergencies
To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
To deliver opportunistic health promotion where appropriate.
