Job summary
We are seeking an enthusiastic and motivated Clinical Independent Prescribing Pharmacist or someone currently studying towards this. As a GP Practice Pharmacist, you will provide patient-facing clinical care, including, advice, treatment and medication management, as well as supporting administrative and operational tasks within the GP Practice.
Main duties of the job
The Clinical Pharmacist will play a key role in supporting safe and effective prescribing with in the practice. Working as part of a multi-skilled team of clinical and non clinical colleagues to deliver high -quality, person-centred and holistic care to patients registered with the practice.
The successful candidate will be an integral part of the clinical team, helping to improve systems, support quality improvement activity and enhance patient experience.

St John's Medical Practice is a friendly forward thinking GP practice based on the outskirts of Sevenoaks in Kent, caring for around 9,000 patients. The practice is led by 2 GP Partners, supported by salaried GPs, ACP, HCA, as well as a skilled administrative team. The practice is part of Sevenoaks PCN and Physiotherapists, Specialist Diabetic Nurses, Social Prescribers and Mental Health clinicians are present at the surgery.

Job responsibilities
Responsible for support to the practice with medication reviews, queries and prescribing safety.
Clinical Services
- Dealing with patient medicine and prescription related queries over the phone, in clinic, via letter.
- Provide independent prescribing services and clinical advice to patients.
- Support medication reviews, reconciliation and optimisation in line with best practice.
- Work collaboratively with GPs, Nurses, ACPs and other primary care professionals to improve patient outcomes.
- Providing a point of contact for the practice for medicines related queries for healthcare professionals and patients.
- Lead and contribute to clinical governance, audit and service improvement initiatives.
Prescription Management
- Reconciliation for medicines from outpatient and discharge letters.
- Working with the practice team to deliver medicines optimisation, prescribing schemes, QOF and locally commissioned services.
- Encouraging, supporting and implementing adherence to local formulary guidance and cost effective prescribing.
- Supporting the pharmacy technician and prescription clerk with repeat prescription processes and medicines management duties
Prescribing Responsibilities
- To have successfully completed the independent prescribing qualification or demonstrate willingness to undertake the course.
Audit and Education
- Conducting clinical audits as part of the multidisciplinary team
- Answering medicines information enquiries from healthcare professionals and patients.
- Implementing systems for monitoring medicines use in conjunction with the practice team.
- Contributing to clinical education of other healthcare professionals.
