Job summary

Township 2 Primary Care Network comprising of Woodhouse Health Centre, Richmond Medical Centre, Charnock Primary Care Health Centre, Stonecroft Medical Centre and Jaunty Springs are seeking to recruit a part time Care Coordinator to work in our expanding network. They will undertake an administrative role, supporting the PCN Manager and practices.

Main duties of the job

As a Care Coordinator you will work across the wider Township 2 Primary Care Network and contribute to the delivery of excellent care.

The Care Coordinator role requires you to build and maintain strong working relationships with professionals, departments and services within the health sector and other related health and social organisations.

Job responsibilities

The post holder will support the primary care network manager and member practices. The post holder should have very good organisational skills.

You will need to work proactively within the team and other teams to ensure integrated working is successful and embedded throughout the Network.

Understands and demonstrate the skills and behaviours required to ensure successful relationships between self and others, including colleagues, patients, clinicians, and other stakeholders

Communication

To tailor communication both verbal and written to suit the audience.

Maintain relevant spreadsheet and databases to support key information for Practices and the wider Network.

To be aware of policies in place around safeguarding adults and children, and recognise you and your team will be required to complete an enhanced Disclosure and Barred Service Check

To adhere to the Networks policies and procedures, including those relating to health and safety; equalities, diversity and inclusion; safeguarding procedures; financial instructions; procurement and GDPR
