This is an opportunity to join our well established friendly practice.

We are fortunate to have a highly skilled and supportive clinical team which already includes two existing full time pharmacists.

We are teaching and training practice, and learning and education are at the heart of everything we do.

We are rated good for CQC with Outstanding in Responsiveness and can offer a competitive rate, and flexible hours.

Main duties of the job

Proving pharmacy duties for a large general practice. This would include long-term condition reviews, repeat prescribing, medication reviews and minor ailments.

We also require a level of support with our COVID-19 medicines delivery unit service which our existing pharmacists help to provide, but full training will be provided for this.

We look after 15000 patients in a large multi-professional dynamic and caring team, including 4 partners, working across our main and branch surgery, and GP Extended Access Hub. Job description

Job responsibilities

KEY RESPONSIBILITIES

Patient facing Long-term condition reviews - Take responsibility for the care management of patients with chronic diseases where medicine optimisation is required. E.g. COPD, asthma, mental health and diabetes.

Telephone and Patient facing clinical medication reviews - Undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care. Medication reviews for patients who are regular A+E, Urgent Care Centre or hospital attenders.

Patient facing medication reviews for housebound patients - Undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Work with carers and care home staff to improve safety of medicines ordering and administration.

Possible future management of common/minor/self-limiting ailments whilst working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals as appropriate.

Management of repeat prescribing and develop standardised protocols across network practices for reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates etc.

Provide specialist expertise in the use of medicines whilst addressing both the public health and social care needs of patients in the Healthsense PCN and help in tackling inequalities.

Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

Take a central role in the clinical aspects of shared care protocols, audit, clinical research with medicines, liaise with specialist pharmacists, community pharmacists and drug monitoring including anticoagulation and DMARDS.

Knowledge, skills and experience required

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

Minimum of 1 years experience as a pharmacist

Must be a Qualified independent prescriber

Have experience and an awareness of common acute and long term conditions that are likely to be seen in general practice.

Recognises priorities when problem solving and identifies deviations from normal pattern and is able to refer as appropriate.

Able to follow legal, ethical, professional and organisational policies and procedures and codes of conduct.

Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

The post holder will supervise him/herself by making decisions regarding meeting targets independently, producing reports highlighting performance information.

Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace

Demonstrate understanding of, and contributes to, the workplace vision

Demonstrate ability to improve quality within limitations of service
