Job summary
Esher Green Surgery is part of NHS Surrey Heartlands and has a patient list of 13,000 patients.
We are looking for a proactive and enthusiastic Pharmacist with prescribing qualifications, preferably with hospital pharmacy experience, to join our Practice to work closely with our nurses and GPs.
The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices
Main duties of the job
As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs. The role would also entail providing primary support to GP staff with regards to prescriptions, medication queries, complex clinical queries - please see full job description.
We will look to provide support to develop and train the candidate to further their skills in their clinical interests. The Pharmacist would have an allocated GP Partner to mentor, support and guide them.
About us
Esher Green Surgery is based in Esher, Surrey. We are a growing, innovative, Practice and we wish to employ our first Pharmacist.
The surgery comprises 3 Partners, 3 salaried GPs, an Advanced Clinical Practitioner, an Advanced Nurse Practitioner, 3 Practice Nurses and a full administration team. Esher Green Surgery is a training practice and we currently host two ST2 and 1 FY2 doctors.

both face to face and via telephone consultations - and will proactively manage
patients with complex poly-pharmacy needs. Review and action Shared Care Agreements,
medicines reconciliations from hospital discharge summaries, audits for
medication safety with a view to making recommendations to amend practice
processes where necessary to improve safety, review and action MHRA medication
safety alerts, carry out virtual medication reviews when reviewing repeat
prescription requests from patients, care homes and pharmacies.

- Qualifications: Pharmacy degree, good command of the English language, team worker, willing to take responsibility, dependable, adaptable. PGDip not essential, independent prescriber not essential, familiar with NHS, knowledge of General Practice not essential.
Experience
Desirable
- Previous primary care experience is desirable but not essential. More important are excellent verbal communication skills, a proactive approach and a desire to learn.
