Main duties of the job
The
Practice Nurse will provide high-quality, patient-centred nursing care within a
small general practice setting. Key responsibilities include long-term
condition management (such as diabetes, asthma, and COPD), health promotion,
cervical screening, immunisations, wound care, and routine treatment room
duties.
The role involves undertaking clinical assessments,
delivering evidence-based care, maintaining accurate clinical records, and
working in line with practice protocols and national guidelines. The Practice
Nurse will work closely with GPs and the wider practice team to support
continuity of care and positive patient outcomes.
As this is a Welsh-speaking practice, the post holder must
be able to communicate confidently in Welsh to support effective patient care
and engagement. The role also includes contributing to infection prevention and
control, safeguarding, and quality improvement activity within the practice.

We are a small, friendly,
Welsh-speaking dispensing general practice serving our local community. Our
team consists of two GP partners, a salaried GP, 2 Physician Associates, 2 Practice Nurse, phlebotomist, and a supportive administrative and dispensing team who work closely
together to deliver high-quality, patient-centred care.
