Job summary

We are seeking a Nurse Practitioner to provide high quality clinical assessment, diagnosis, treatment, and evaluation for patients presenting with a wide range of acute and long term conditions. The successful candidate will work autonomously as well as collaboratively with our multidisciplinary team.

Main duties of the job

  • Provide clinical assessment, diagnosis, and treatment within your scope of practice
  • Manage patients presenting with undifferentiated, undiagnosed conditions
  • Prescribe medications and manage treatment plans
  • Support the practices urgent care/on-the-day demand
  • Work closely with GPs, ANPs, Practice Nurses, HCAs, and other allied health professionals
  • Maintain accurate and timely clinical records

Montague Medical Practice provides services to a registered population of approximately 9000 patients, in the town of Goole and surrounding areas. The post holder will provide assessment and care for patients presenting with routine, urgent or acute clinical needs. This includes both telephone triage and face to face consultations. They will independently assess, diagnose, treat, and manage patients presenting with minor illness, minor injury, or acute exacerbation of chronic conditions, referring to investigations or external services when required.

The Nurse Practitioner will work autonomously within their professional competencies, seeking support from the clinical team as appropriate. Excellent clinical judgement, prioritisation skills, and safe practice are essential to the role
