Job summary
WEB Primary Care Network (PCN) are looking for a Social Prescriber to join the PCN Mental Health & Wellbeing (MH&WB) Team.
The MH&WB service aligns with and supports the Community Mental Health Team (CMHT) so the post holder must have the confidence to work across Primary, Secondary and Community services and the ability to develop strong working relationships. You will be expected to work at various locations across the PCN as appropriate.
Primary Care is challenging yet rewarding and developing to offer new opportunities. We are looking for someone who loves a challenge and is adaptable to this developing service that is kept under constant review and refreshed in response to changing needs.
You will have the benefit of being part of a large, high function group whilst being part of a new and small developing team.
Main duties of the job
Your role within this team will include assessing individual patients and managing a patient list; providing signposting to different services and connections with community groups and services to improve general health and well-being to people across our practice population. These services may include befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering, forging working relationships and creating referral pathways with VCSE organisations.
You will be confident to work 1:1 with patients in a variety of settings, in their own homes, the community or in GP surgeries; using behaviour techniques such as motivational interviewing to support them to identify, set and meet achievable goals to improve their physical and emotional wellbeing.
Make and maintain contact with local health and wellbeing service providers forging working relationships and building a network and knowledge of referral routes to and from service providers. Ensuring information about providers and voluntary groups is shared within the MH&WB Team.

Social Prescribers
work with a patient on what matters to
the person as identified through shared decision making or personalised care
and support planning. They can connect people to community groups and agencies
for practical and emotional support.
It can
especially help people who:
- have one or more long-term conditions
- need support with their mental health
- are lonely or isolated
- have complex social needs which affect their wellbeing
Social prescribing
links them to a range of activities that are typically provided by voluntary
and community sector organisations, for example, volunteering, arts activities,
group learning, gardening, befriending, cookery, healthy eating advice and a
range of sports.
Social Prescribers
give people time and focus on what matters to the person as identified through
shared decision making or personalised care and support planning. They can
connect people to community groups and agencies for practical and emotional
support (The National Academy for Social Prescribing).
They work within the multidisciplinary Mental Health & Wellbeing (MH&WB)
Team, embedded within the PCNs and collaborate with local partners to support
and empower patients to manage their needs.
Duties and Responsibilities
The Social Prescriber will:
- Take referrals from GPs, practice
nurses, MH&WB team members including DPT colleagues within the
team. In future referrals may be
taken from a wide range of agencies i.e. networks members, pharmacies,
multi-disciplinary teams, hospital discharge teams, allied health
professionals, fire service, police, job centres, social care services,
housing associations and voluntary, community and social enterprise (VCSE)
organisations.
- Provide personalised support to
individuals, their families and carers to take control of their wellbeing,
live independently and improve their health outcomes.
- Develop trusting relationships by giving
people time and focus on what matters to them.
- Take a holistic approach, based on the
persons priorities, and the wider determinants of health.
- Co-produce a simple personalised care
and support plan to improve health and wellbeing, introducing or
reconnecting people to community groups and statutory services.
- Evaluate the individual impact of a
persons wellness progress.
- Manage and prioritise their own
caseload, in accordance with the needs, priorities and any urgent support
required by individuals on the caseload.
- Where required and as appropriate, refer
people back to other health professionals within the network.
- Draw on and increase the strengths and
capacities of local communities, enabling local VCSE organisations and
community groups to receive social prescribing referrals. They will ensure
those organisations and groups are supported, have basic safeguarding
processes for vulnerable individuals and can provide opportunities for the
person to develop friendships, a sense of belonging, and build knowledge,
skills and confidence.
- Have a role in educating non-clinical and clinical staff
within the network on what other services and support is available within the
community and how and when patients can access them. This may include verbal or
written advice and guidance.
- Promote client involvement in the management of
the service

Organisational Position
The Social Prescriber will report to WEB PCNs Project Manager.
They will also be required to report to the GP Practice Leads where they are based.
Individual Responsibilities

- Participate in
reflection, self-evaluation and continuous professional development including
performance review. Ensure mandatory
training is up-to-date at all times and participate in meetings as required.
- Maintain administration systems and workspace used
in a clean and tidy condition.
- Maintain a good working knowledge of Health and
Safety procedures and fire precautions, and operate the correct procedures and
participate in policy development and data collection where appropriate.
- Work flexibly to meet the needs of clients.
- Ensure mandatory training is up-to-date at all
times.
- Contribute to ongoing/new projects as required.
- Undertake any other duties appropriate to the grade
and purpose of the job.
Work Setting and Review
The post holder will work to an agreed set of
targets and objectives, as both a member of the Social Prescribing team and
overarching MH&WB service. There will be a mix of autonomous and team
working.
They will manage their own time and case load,
providing regular updates/progress reports to their line manager and to the GP
practices they are based in.
