We are a supportive practice with a close-knit doctor team. A long-standing training practice where support and development are very important to us across all areas of the Practice. We support the GP Fellowship scheme.

With a wide clinical team of paramedic, clinical Pharmacists, Nursing team, HCAs and a large Patient Services Team including receptionists, secretaries, coders and summarisers.

We use Emis Web, Ask First, AccuRx, and Docman

Main duties of the job

On a full day (non-duty) normally up to 22 patient contacts (phone/F2F), with each GP having a Gold card appointment for use by them to prebook patients in that need extra support. Meds management and paperwork have a cut off time.

On a half day 10 patients and again cut off times for paperwork and meds management

Navigation -In August 2023, we implemented a new system where every morning, one of our GPs is based alongside our highly trained patient services team (PST), ensuring the correct care is provided and that appointments are made efficiently.All appointment requests are clinically triaged by our GP. This ensures the right appointment is offered to suit the patients needs.

Some benefits of Navigation :-

  • We are offering more pre-bookable appointments, either face to face or a telephone call
  • We are able to deal with quick wins like out of stock medications, prescription approval and signposting to other services when a GP appointment is not necessary.

On a duty day you will work as part of the duty team 08:00-18:30 along with one other GP, the paramedics and minor illness nurse dealing with the acute patients on the day. Duties are allocated on a pro rata basis.

We also offer extended hours and these sessions are paid at an enhanced rate and these are optional to sign up to

We support all our new GPs with providing them with a mentor and all GPs have a buddy.

We have a daily duty huddle where the duty team and management team plan the day

Supporting the wellbeing of our staff is a very important part of the Poplar Grove ethos and work life balance is crucial. We have a number of Practice socials throughout the year. We value input from all the staff and we recognise their participation in making Poplar Grove Practice work.

As well as Protected Learning Time which is provided by the ICB for ten afternoons a year, the Partners support monthly specific clinical education lunchtime meetings

Job summary:

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. As a key member of the clinical team you will provide primary healthcare services to patients in all clinical settings in addition to actively participating in practice development activities including clinical and staff meetings, audits and relevant CPD.

Clinical responsibilities:

  • In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • Developing care plans for health in consultation with patients and in line with current practice disease management protocols
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards

The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include (but will not be limited to):

  • Using personal security systems within the workplace according to practice guidelines
  • Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
  • Providing advice on the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
