Job summary

Islington Central Medical Centre is looking for an experienced care co-ordinator to join our multidisciplinary team. A care co-ordinator is expected to work closely with GPs, practice teams, and the wider multi-disciplinary team to support patients, particularly those who are frail, elderly, or living with long-term conditions. By helping these patients co-ordinate their care, care co-ordinators help patients in navigating health and care services, and ensure that patients and their carers receive the right support. Moreover, care co-ordinators act as the central point of contact for patients, making sure that the evolving needs of both the patients and their loved ones are well met.

Islington Central Medical Centre is a large teaching and training practice with almost 25,000 patients and a large team of clinical and administrative staff. ICMC is a purpose-built surgery, located just a 5-minute walk from Highbury and Islington Station, which has excellent underground, overground and national rail transportation links.

Main duties of the job

Care Co-Ordinators:

  • Improve patients quality of life
  • Work in a patient facing role to assist the practice with its caseload
  • Review patients needs and help them to access appropriate services
  • Ensure that the patient has holistic care by co-ordinating with other members of the MDT, including social prescribers, nurses, mental health services and other professionals
  • Support patients as they make critical decisions and centre and personalise their care around their needs
