Job summary

We are excited to offer a fantastic opportunity for a dedicated and forward-thinking General Practitioner to join our team atChase Meadow Health Centre, as part of theAdditional Roles Reimbursement Scheme (ARRS)withinKenilworth and Warwick Primary Care Network (PCN). This is an innovative position designed to enhance integrated care and expand capacity across our neighbourhood.This role is ideal for a GP seeking variety, collaborative working, and the chance to deliver high-quality care within a supportive and well-established team. Youll work closely with a range of ARRS professionals including Clinical Pharmacists, First Contact Physiotherapists, Social Prescribers, and Health and Wellbeing Coaches alongside our practice staff to improve outcomes for our local population.

Main duties of the job

Job DescriptionAs a GP working under the ARRS scheme at Chase Meadow Health Centre, your role will contribute to the development and delivery of enhanced primary care services in our community.Key responsibilities:
  • Provide holistic, person-centred care to a defined patient list, with protected time to focus on continuity and proactive management.
  • Contribute to the delivery of the GMS core contract specifications including QOF, Enhanced Health in Care Homes, Anticipatory care, and Personalised care.
  • Work as part of a multi-disciplinary team within the practice and PCN, supporting integrated working with Acute, community, social care, and voluntary sector partners.
  • Participate in quality improvement activities, data audits, and clinical supervision where appropriate.
  • Engage in regular meetings and neighbourhood-level planning for population health management.
  • Promote safe prescribing practices and contribute to medicine optimisation where relevant

  • GMC registration and inclusion on the GP Performers List
  • Fully qualified GP with MRCGP or equivalent
  • Within 2 years post-CCT
  • Eligibility to work in the UK
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively within a team
  • Commitment to patient-centred care and service improvement

Desirable

  • Experience of working within a Primary Care Network or multi-disciplinary team
  • Interest or additional qualification in chronic disease management, frailty, or preventative care
  • Familiarity with EMIS Web (clinical system)
  • Previous involvement in quality improvement or audit work
