Clinical Pharmacist - Maternity Cover 12 months

Job summary

We are seeking an experienced and motivated Clinical Pharmacist to play a pivotal role in delivering high-quality, patient-centred medicines management within our general practice. Working as clinicsl pharmacist, you will provide expert clinical input through medication reviews, face-to-face and telephone consultations, and support for patients with acute and long-term conditions. You will lead on safe, effective and cost-conscious prescribing, optimise repeat prescribing systems, and support the multidisciplinary team to improve patient outcomes, safety, and service efficiency, in line with best practice, NICE guidance and strategic objectives.

Main duties of the job

  • Conduct medication reviews, reconciliation, and targeted reviews for high-risk patients

  • Implement and manage safe, effective repeat prescribing systems

  • Lead on medicines optimisation, cost-effective prescribing, and safety alerts

  • Support vulnerable patients and improve medicines compliance

  • Provide expert medicines advice to patients and the multidisciplinary team

  • Ensure prescribing aligns with clinical guidance and quality standards

  • Contribute to audits, service improvement, and clinical education within the practice
