Social Prescribing Link Worker for Young People

Job summary

We are looking to recruit a Social Prescribing Link Worker with a focus on Children and Young People. The role will be aligned to Trafford West Primary Care Network, a multi-disciplinary healthcare team.

The role will involve an element of 1 -1 personal support for children and young people with an emphasis on enabling them to access additional community based, person centred support that meets their physical, social and emotional needs. This role is primarily to work with children and young people aged 5 -18 years within a holistic, person-centred community-based framework.

Main duties of the job

Main tasks

To work with children and their families to assess, discuss and refer into appropriate support services offering community based mental wellbeing advice and support.

To liaise with the parents to navigate and VCFSE services and sign to online services such as Kooth, youth groups and other locally based services.

Key Objectives

To engage, facilitate and support young people in their development, connecting them into activities to ensure they experience:

Reduced feelings of loneliness and isolation supported to build positive relationships and improved connections with their community

Improved engagement into a range of activity and social opportunities self motivation to set personal goals and develop aspirations

Improved understanding of risks to their overall wellbeing / manage and reduce personal risk increased ability to make positive choices to keep safe and well.

Key areas of Responsibility

To receive and process referrals from West Primary Care Network and other third parties

Provide personalised support to children and young people to enable them to take control of their wellbeing, live independently and improve their health outcomes, appropriately supported by family or carers

Take a holistic approach, based on the child or young persons priorities and the wider determinants of health.

Co-produce a personalised support plan to improve health and wellbeing, introducing, connecting and supporting young people to community groups and statutory services.

To meet with and provide support to children and young people at home, school or within designated community spaces.

To build positive, trusting, non-judgemental relationships with young people, enabling them to engage in appropriate services and interventions and make progress towards and achieve their goals.

To develop and facilitate a range of groups and activities young people to attend which enables them to grow in confidence, develop coping mechanisms and gain additional skills and experience to build resilience.

To promote and ensure the safeguarding of children and young people, particularly those who may be identified as vulnerable or at risk of harm; and to follow up on all safeguarding concerns.

Build relationships with key staff in GP practices within the West Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, promoting and raising awareness of Social prescribing, and supporting practices in attaining their social prescribing referral targets.

To collect, collate and input data into Primary Care Network monitoring and evaluation collection databases.

To provide regular written and spoken reports and analysis of work to the as required.

Adhere to data protection legislation and data sharing agreements within the Primary Care Networks.

To demonstrate a commitment to continuing professional development and to attend appropriate training.

To work flexibly including some evenings and weekend delivery, to ensure that the needs to the young people are met.
