You will operate under the direction of The Counselling & Family Centre (CFC) Adult Services Manager and the PCN Clinical Director.
Social Prescribing is a free service to help people improve patients general health and wellbeing in a holistic way. Through using non- medical sources of community support, Social Prescribing offers the chance to find out about activities that can improve health and wellbeing, including advice and information services, community groups, leisure activities, lunch clubs, self-help groups, specialist interest groups, sporting activities, and lots more. As a 1st contact link worker for young people, you will be working closely with the GPs, and taking referrals from the surgeries.

Main duties of the job
To work with children and their families to assess, discuss and refer into appropriate support services offering community based mental wellbeing advice and support.
To deliver brief intervention therapies when identified as appropriate, along with work- books and self- help guides.
To Advise and support education staff and signpost further information to ensure they can give children and young people the right support.
To liaise with parents and Carers to navigate VCFSE services and sign to online services such as Kooth, youth groups and other locally based services.
To engage, facilitate and support young people in their development, connecting them into activities to ensure they experience:
Reduced feelings of loneliness and isolation supported to build positive relationships and improved connections with their community
Improved engagement into a range of activity and social opportunities self motivation to set personal goals and develop aspirations
Improved understanding of risks to their overall wellbeing / manage and reduce personal riskincreased ability to make positive choices to keep safe and well.

Traffors West PCN is made up of 6 GP Practices are working together to provide proactive, personalised, co-ordinated and integrated health and social care to the community of Trafford West. We serve 54000 patients. We have two Clinical Directors, a PCN Manager and 20 other various staff roles.
You will work across the various sites mentioned in the Job location.
We work together with the community, mental health, social care, pharmacy, hospital, and voluntary services in our local areas.
Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan and play a fundamental role in making sure we get the best care!
