Job summary

We are looking for a Care Co-ordinator to work closely with GPs and practice teams at the North Oxfordshire Rural Alliance (NORA) PCN for 28 hours per week to assist with case-finding and pathway management of patients with potential Hypertension, AF or Familial Hypercholesterolaemia. The primary purpose of this role will be to assist NORA PCN in reducing the impact of cardiovascular risk factors and to improve awareness and identification of heart failure.

This is an administrative role, but any previous clinical experience, especially in the area of cardiovascular disease and its management, would be an advantage but it is not essential.

This role requires remote working but there may be opportunities to spend time in any of the 5 NORA PCN practices. We are flexible in terms of when the 28 hours per week are worked but this role cannot be performed beyond normal office hours (ie prior to 08.00 or after 18.30). As travel to some of our practices cannot be done on general bus routes, the successful candidate will need to be able to drive and to have access to a reliable and road-worthy vehicle.

The salary will be up to £12.50 per hour dependant on experience

Main duties of the job

Primary Duties and Areas of Responsibility

Using existing patient medical records, identify patients with potential Hypertension or Hypercholesterolaemia.

Contact patients to start a diagnostic pathway via a number of means which might include:

o Further monitoring by the patient at home advise on approved BP monitors

o Further BP checks in a community pharmacy or other community location

o Instructing the patient to use waiting room machines to re-check their BP

o If none of the above are suitable, asking the patient to book in to their practice for a BP check

Using the approved pathways, ensure that patients with a likely diagnosis of hypertension are guided through the diagnostic process in the most efficient way possible utilising existing practice staff and resources (or additional PCN staff)

Chase up patients to ensure that any further monitoring/diagnostic tests are undertaken in a timely manner

Build links with local community pharmacies and community spaces to build diagnostic capacity in non-healthcare settings

Keep NORA PCN practices informed of all findings/data

The Cardiovascular (CVD) Care Coordinator will play an important role within our Primary Care Network, working with a local cluster of General Practices as part of North Oxfordshire Rural Alliance (NORA) Primary Care Network (PCN). NORA PCN Practices are at Bloxham & Hook Norton Surgery, Chipping Norton Health Centre, Cropredy Surgery, Deddington Health Centre and Wychwood Surgery.

b. Ensure any issues are communicated to the GP and any other professionals involved in the persons care in a timely manner and uploaded to the relevant online care records

2. Data and information capture

a. Keep accurate and up-to-date records of contacts, appropriately using GP and other records systems relevant to the role, adhering to information governance and data protection legislation;

b. Record and collate information according to agreed protocols and contribute to evaluation reports required for the monitoring and quality improvement of the service.

3. Professional development

a. Work with a named clinical point of contact for advice and support.

b. Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning activity as required;

c. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety.

4. Miscellaneous

a. Establish links with local community pharmacies and community spaces to build diagnostic capacity in non-healthcare settings

b. Act as a champion for CVD and blood pressure checks within the PCN;

c. Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner;

d. Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning;

e. Contribute to the development of policies and plans relating to equality, diversity and reduction of health inequalities;

f. Work in accordance with the practices and PCNs policies and procedures; g. Contribute to the wider aims and objectives of the PCN to improve and support primary care;

Health and Safety/Risk Management

The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
