Are you an experienced and motivated Advanced Nurse Practitioner, or Advanced Clinical Practitioner looking to make a real difference in Primary Care?
We are seeking a dynamic and patient focused ANP/ACP to join our friendly, rural practice at Hartington Surgery.
35 hours per week including extended access.
This is a permanent position for an experienced practitioner to work alongside our established clinical team. Job share between 2 applicants may be considered.
Interviews are scheduled to take place on Wednesday 20th August.
Main duties of the job
Professional registration and Master's Degree in Advanced Clinical Practice (or equivalent), or working towards, in addition to a non-medical prescribing qualification.
Key responsibilities:
- Candidates must be able to work autonomously conducting clinical assessments, diagnosing, treating and evaluating care for patients
- Ability to deal with the wide range of patients who present to primary care
- Independently manage a range of acute and chronic conditions including patients with complex needs in line with evidence-based practice and QOF requirements
- Lead on health promotion, disease prevention, and management of long term conditions
- Provide clinical advice and mentorship to junior clinical staff
- Undertake home visits for patients as required
About us
Hartington Surgery is a small but busy, rural General Practice in the heart of the Peak District
National Park, close to the Derbyshire, Staffordshire border. Hartington Surgery is part of the High Peak Primary Care Network (PCN), working closely with our Network Partners and benefiting from a PCN MSK service, Pharmacy Team, Social Prescribing Service, Dementia Support Service, Cancer and Palliative Support Services alongside Home Visiting Services.
The Practice is fully dispensing and
holds a General Medical Services contract to provide a range of medical care to
approximately 3500 patients. The varied population comprises of patients employed in farming,
quarrying, leisure and tourism as well as being a central location for patients commuting to towns
and cities in each direction.

- Manage undifferentiated undiagnosed conditions and identify red flags / underlying serious pathology and take appropriate action
- Use complex decision making to inform diagnosis, investigation and complete management of episodes of care within a broad scope of practice
- Ensure patients receive high quality care, delivered in a timely manner
- Ensure complete and accurate documentation of each patient contact
- Undertake home visits as required
- Independently prescribe within the scope of professional practice
- Request and interpret investigations and act on results appropriately
- Support management of long-term conditions in line with evidence-based practice and QOF requirements
Applicants should be experienced practitioners who act within their professional boundaries providing care from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, demonstrating critical thinking in the decision making process.
Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing mentoring and leadership, as required.
