Job summary

Data Entry Clerk required at GP Surgery in Stowmarket – 15 hours a week Monday and Thursday 0900 to 1700 hrs plus holiday and sickness cover. Must be computer literate with attention to detail.

MAIN PURPOSE OF THE POST: To collate, edit and enter data onto IT systems in accurate and timely manner

Job description

Job responsibilities

Administrative Duties:

· To enter accurate and timely clinical data onto practice IT systems to monitor information on SystmOne (or similar system) and compile reports as requested.

· To manage the pathology results and file specific results as per the pathology protocol and ensure this is dealt with in a timely and accurate manner.

· To transfer the clinic letters received via email from hospitals or other to systmone for processing through document manager/scanning

· To process and read code clinic letters and discharge summaries and ensure all information is dealt with in a timely and accurate manner as per protocol.

· To scan and process paper correspondence on to the patient records in accordance with coding protocol in an accurate and timely manner

· To maintain patient’s registration details by entering new patient details from new patient questionnaire and scan on to Systmone

· Enter daily BP results on to patient record and calculate 2day HBPM results and follow up outcomes as per protocol

· To record Sharing consent, Summary Care Records and Additional SCR on patient record

· Enter cervical smear results on to patient record and follow up the non-responders via open Exeter system.

· To action Out of Hours and ensure all information is dealt with as per protocol

· Accurately enter vaccinations and childhood immunisations onto the patient record

· To monitor information on SystmOne to ensure QOF targets are being met and compile reports and updates as requested

· To collate and accurately enter chronic disease management information for LTC ensuring recalls are kept up to date.

· To record patient details on the Retinal screening hub data base for diabetic patients

· Summarise new patient notes as per protocol

· Maintain the practice carer register

· Action and process tasks received from third parties

· To action the degraded data relating to GP2GP transfers

· To undertake clinical searches, audits and statistics as requested
