Job summary
East
Suffolk Primary Care Network is an innovative partnership of 12 practices
providing primary healthcare to more than 128,000 patients. The collaboration
sees doctors and other practice staff sharing resources and working together to
ensure patients continue to receive the highest quality healthcare.
You
will work as part of the Pharmacy team to support them with administrative work
which will include regular contact with patients and support the delivery of
the Pharmacy service.
Main
duties include:
Using
clinical system searches-mostly pre-written- to proactively identify and work
with target cohorts of patients for structured medication reviews and safety
audits reviews
Contacting
patients via telephone, e mail and SMS to facilitate appointment booking and
engagement with care and treatment.
Liaising
closely with the practice teams
Hours - up to 30 per week
Interviews will be held on Tuesday 17th October 2023. The interview panel will consist of a minimum of 2 people.
