Job summary
Comprising of Derby Road and Pinewood Surgeries, Derby Road
Practice is an inclusive, friendly, flexible, forward thinking,
multi-disciplinary primary care team looking to recruit a PCN paramedic to join
our expanding PCN team. As a training practice,
we will support, train and professionally develop the right individual who can
help us meet our patients’ needs.
Our surgeries have a relatively low visit request rate, so
although we need the ability for you to visit patients in their own homes, your
main work will be minor illness clinics held on site. You will be supported by our teams of
experienced GPs, Advanced Nurse Practitioners and practice Pharmacists. We have recently introduced the Ask My GP
online patient consultation portal which gives clinicians the flexibility to
plan and manage their own day.
A prescribing qualification would be desirable though not
essential. We would very much support
any candidate wishing to obtain a prescribing qualification.
Main duties of the job
To provide a high-quality, cost-effective service to the patients
of the Primary Care Network (PCN); diagnosing and prescribing treatments and medication
while working within defined procedures and protocols as laid down by the GPs.
To work as part of a clinical team, with GPs, Advanced Nurse
Practitioners, Mental Health Practitioners, Practice Nurses and Healthcare
Assistants across practices in the PCN.
To be responsible for seeing patients across multiple practice
sites and for occasionally visiting patients in their place of residence, who
present with a wide range of minor illness, urgent and same day requests for
treatment and chronic disease management; providing care for the presenting
patient from initial history taking, clinical assessment, diagnosis, treatment
and evaluation of care.
The post holder will need to demonstrate safe, clinical
decision-making and expert care, including assessment and diagnostic skills.
There will be the need to prioritise and triage the needs of patients
accordingly making any necessary referrals for investigations in the
appropriate manner.

- Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs either in the practice sites or patients’ homes. Act as an autonomous clinician, with the support and guidance from the GPs
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
- Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
- Recognise, assess and refer patients presenting with mental health needs
- Telephone triage as appropriate for clinics and home visits and provide access to GPs as necessary.
- Evaluate clinical information from examination and history taking and initiate appropriate treatment and / or referral, including admission to hospital if required.
- Signpost appropriately to other services in the identified healthcare pathway and to refer patients to appropriate care settings, within locally agreed healthcare pathways.
- Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
- Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
- Provide clinical advice to other members of the team where appropriate.
- Support the GPs in other duties as required for example assisting with ECGs or other nursing duties where a nurse is not available.
