Haven Health Practice is an inclusive, high achieving, friendly, flexible, forward thinking, multi-disciplinary primary care team. As a training practice we will support, train and professionally develop the right individual who can help us meet our patients’ needs.
Haven Health, part of Suffolk Primary Care looks after 9000 patients. Based in Felixstowe on the east coast, with close travel links to the beautiful Suffolk countryside and leading towns.

Main duties of the job
* We use the AskmyGP online consultation service – allowing us to manage our own workload with control over face-to-face appointment lengths and timings.
* Our team of doctors and advanced nurse practitioners deal with the majority of daily clinical enquiries.
* We hold long-term conditions clinics weekly, utilising the skills of our fantastic HCA’s and nurses.
* We utilise a streamlining process for pathology results and letters to reduce the number of administrative tasks per day.
* Our practice pharmacist, prescription clerks and PCN pharmacy team deal with the majority of medication queries.
* Haven Health paramedics carry out all our house bound patient visits daily
* Finally, we have an Allied Health Professional physio service to triage MSK problems.

Clinical Responsibilities:
* In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient test results, reports and other paperwork and correspondence in a timely fashion
* Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
* Assess the health care needs of patients with undifferentiated and undiagnosed problems
* Screen patients for disease risk factors and early signs of illness
* In consultation with patients and in line with current practice disease management protocols, developing care plans for health
* Provide counselling and health education
* Admit or discharging patients to and from the caseload and referring to other care providers as appropriate
* Record clear and contemporaneous consultation notes to agreed standards
* Collect data for audit purposes and complying with QOF
* Compile and issuing computer-generated acute and repeat prescriptions
* Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
* Participate in teaching and supporting medical students
* In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
