Job summary

Receptionist/Care Navigator Required at Brandon Medical Practice

In the region of 20 hrs a week - 2 x part time roles available:

Post 1: £10,167 (19.5 hours per week)

Mon: 8am - 2pm

Tues: 1pm - 6:30pm

Weds: 1pm - 6:30pm

Thurs: 8am - 12 midday

Post 2: £10,298 (19.75 hours per week)

Mon: 2pm - 6:30pm

Weds: 8am - 12 midday

Thurs: 1:45pm - 6:30pm

Fri: 8am - 12 midday

Main duties of the job

Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

Must be available to cover shifts between 7.45 am and 8.00 pm Monday to Friday, some Saturday working will be required.

Be available for cover of other reception team members due to sickness and annual leave

Computer Literacy is a must but training will be provided for all other areas.

Customer service experience is desirable and to have a willingness to help others.

The post will be subject to suitable references and DBS clearance.

Duties include but are not exclusive to.

*Greeting and booking in patients for surgeries and clinics

*Dealing with patient enquiries efficiently and courteously

*The receiving and recording of telephone messages taking appropriate action

*Making appointments on our clinical system, SystmOne.

*Taking home visit requests

*Receiving specimens from patients ensuring all their details have been recorded whilst following Health & Safety policy.

*Provide admin support for the clinical team

*Scanning documents into the clinical system.

*To undertake any other duties as deemed appropriate by the Practice

*To have a thorough knowledge of all practice procedures and work in accordance with written protocols.

* Ensure building security at the end of the day.

SUMMARY OF MAIN DUTIES: The post-holder will project a positive and friendly image to the surgery patients and other visitors, deal with queries in a professional, courteous and efficient way.

DUTIES & RESPONSIBILITIES:

· A Care Navigator helps to facilitate the patient’s journey

· This journey can take many different pathways and can help manage the workload

· They give specific and poignant information and provide the best options

· They provide help and direction and consider more appropriate services

· They can signpost the patient to dispensary, secretaries, clinical pharmacist, nurse

Led team, nurse practitioners, doctors and community pharmacists

· They can identify emergencies and prioritise calls for the clinicians, having gathered

Information from the patient

· The Care Navigators are also able to support the secretarial team by arranging

Blood tests, ECG’s, follow up telephone calls to the GP’s, sending out letters for blood pressures etc

· To answer telephone calls and over the counter enquires from patients

· To book nurse appointments

· To arrange 24 hour ECG’s

· Registrations and Temporary Residents

· Chaperone

· Results and hospital letters

· New patient notes in and out of the surgery
