Job summary
We would like you to join our dynamic and passionate team as a Clinical Pharmacist to provide safe, quality, pharmacy services in primary care, working with a wide range of colleagues within Foundry Primary Care Network. You will have the opportunity to develop and deliver safe, effective, and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. In this role you have the potential to help manage general practice workload and significantly improve quality of care and safety for patients.
Main duties of the job
As a Clinical Pharmacist within the Foundry Primary
Care Network you will contribute to the delivery of excellent patient care
To provide Medicine
Information to practice
staff and patients.
Conduct consultations face to face / via phone for
patients with long term conditions, polypharmacy, which may include deprescribing
or up-titration of medication. Review the on-going need for each medicine,
a review of monitoring needs and an opportunity to support patients
with their medicines
taking ensuring they get the best use of their medicines (i.e. medicines optimisation).
Work as part of the wider clinical
team (including Pharmacists, GPs, nurses, Social Prescribing Team, Physician
Associates etc)
Repeat prescribing
- Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines
reaching review dates and flagging
up those needing a review and monitoring.
Risk Stratification - Identification of cohorts of patients at high risk of harm from medicines
through pre-prepared practice
computer searches.
Medicines quality improvement
Medicines safety
Implementation of local and national guidelines
and formulary recommendations
Care Quality Commission - Work with the general practice team to ensure the practice is compliant with CQC standards
where medicines are involved.

Pharmacist who will provide on going coaching, development and support.
The post holder will work as part of a
multi-disciplinary team in a patient-facing role. The post holder will take
responsibility for areas of chronic
disease management, areas of the
quality and outcomes framework and elements of CQC compliance within the
practice and undertake clinical medication reviews/ structured medication
reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary
support to general practice staff and patients with regards
to prescription and medication queries. They will help support the repeat prescription system, deal with
acute prescription requests, and medicines
reconciliation on transfer of care and systems for safer prescribing, providing
expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
The

Qualifications
Essential
- Masters degree in pharmacy (MPharm) and minimum of two years post-qualification experience.
- Completed the CPPE primary care pathway or be working towards it or willing to work towards.
Desirable
- Independent Prescribing Qualification

- Drive and determination to promote and support the role of pharmacy staff in General Practice
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
- Demonstrates the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
- Is able to plan, manage, monitor, advise and review general medicine issues in core areas for long term conditions
- Able to work under pressure and to meet deadlines
- Work effectively independently and as a team member
- Understands the principles of equality and diversity and commitment to addressing health inequalities and patient empowerment
- Experience of providing and using information to support the delivery and performance management of services
