Main duties of the job

Key Responsibilities:

Assess and manage patients with undiagnosed, undifferentiated medical conditions and make treatment decisions using complex decision making, including ordering necessary investigations, following appropriate guidelines and policies

Collaborate with the wider healthcare team and be involved in education and quality assurance processes

Refer patients to secondary care and other holistic therapies

Undertake appropriate home visits and Care Home ward rounds, and complete Care Plans and ReSPECT forms when required

Provide high-quality, patient-centred care

Requirements:

Advanced Clinical Practice qualification (Masters Level)

Independent Prescribing qualification or working towards this qualification

Current registration with the relevant professional body

Excellent communication and interpersonal skills

Job responsibilities

The ideal candidate might be from a variety of backgrounds such as an advanced nurse practitioner, pharmacist, or paramedic and must hold a masters degree in a relevant area of advanced practice

Key Objectives

When running your own clinics:

o Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team

o Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action

o Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice

o Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person

Safety

Report medicines related incidents, contribute to investigations and root cause analyses

Participate in serious incident investigations and multidisciplinary case reviews

Report Adverse Drug Reactions and promote reporting by MDT

Action and respond to Drug Safety Alerts and other safety cascades.

Quality

Ensure compliance with CQC and other professional standards

Promote a culture of constant improvement and excellence

Conduct regular clinical audits and other improvement activities

Assess own performance regularly and make suggestions of how to improve personal and team performance

Health & Safety

Assist in promoting and maintaining your own and others health, safety and security as defined in the Practice Health and Safety policy. Ensure that any hazards in your working area are reported immediately to your line manager.
